Clinical Characteristics And Related Factors Analysis Of Multipl Myeloma Patients With Renal Impairment

Objective Retrospectively analysis the clinical data of 150 cases with newly diagnosed Multiple Myeloma(MM),in order to study clinical characteristics and related risk factors of MM patients with renal impairment.Methods The clinical data of 150 newly diagnosed MM patients in Gansu Provincial Hospital from January 2015 to January 2021 were statistically analyzed.According to whether the serum creatinine value of newly diagnosed Multiple Myelomaat was less than 177umol/ L,MM patients were divided into two groups: normal renal function group and abnormal renal group.The clinical characteristics and risk factors of renal impairment were analyzed and compared between the two groups.Chi-square test was used for comparison of enumeration data,t-test was used for the comparison of measurement data.The independent risk factors for multiple myeloma with renal function impairment in MM patients were analyzed by binary Logistic regression.Results Among the 150 newly diagnosed MM patients,there were 110cases(73.33%)with normal renal function,40 cases(26.67%)with renal impairment,and 103 cases(68.67%)with male patients and 47 cases(31.33%)with female patients.The mean age was(61.45 ±9.71years)years old.The ratio of male to female was 2.19:1.Red blood cells,white blood cells,hemoglobin,serum calcium,serum phosphorus,corrected serum calcium,serum uric acid,serum cystatin C and serum β2microglobulin were compared between the two groups,and the differences were statistically significant(P < 0.05).Univariate binary Logistic regression analysis showed that red blood cells,hemoglobin,white blood cells,serum uric acid,serum calcium,corrected serum calcium,serum phosphorus,serum cystatin C,and serum β2microglobulin were influential factors in renal function impairment in MM patients.The statistically significant data obtained from univariate analysis were included in multivariate Logistic regression analysis,which showed that serum β2microglobulin was an independent risk factor for renal function impairment in MM(P<0.05,OR>1).Conclusions1.The top three clinical manifestations of 40 patients with renal function impairment were lumbar and back pain,lower limb edema and dizziness and fatigue.2.Most of the150 newly diagnosed MM patients entered stage Ⅲ and the Ig G is the most,and most of the 40 patients with renal function impairment entered stageⅢ,and the light chain type is the most.3.The serum β2 microglobulin is independent risk factors for renal impairment in newly diagnosed MM patients.
