| Objective:To explore the effect of Roy diet in hemodialysis patients,and provide reference for clinical nursing work.Methods:1.Taking the theory of KAP as the framework,we developed the KAP questionnaire for major caregivers of MHD patients,tested the reliability and validity of the questionnaire and applied it in the experiment.2.By convenience sampling,120 hemodialysis patients were selected from the blood purification center of a Grade-A hospital in Lanzhou city.They were randomly divided into control group(n = 60)and experimental group(n = 60).The control group adopted the traditional nursing procedure to educate the patients and their caregivers on diet,while the experimental group assisted the patients and their caregivers to make a diet plan under the guidance of Roy adaptation model,so as to improve the nutritional status of the patients.Before the start of the experiment and 6 months after the dietary intervention,the self-made questionnaire of dietary nutrition knowledge,attitude and practice of main caregivers of maintenance hemodialysis patients,subjective nutritional status assessment form,dietary compliance assessment form and quality of life assessment form of patients with kidney disease were used to evaluate twice.The changes of albumin and hemoglobin values of patients before and after the dietary intervention were detected,and the intervention was analyzed effect.Results:1.There are 39 items in the questionnaire,the Cronbach’s α level of the questionnaire is0.78;the test-retest reliability(R)is 0.82;the I-CVI of the questionnaire is 0.89,S-CVI is0.91,and the total CVI is 0.90;the results of exploratory factor analysis show that KMO =0.79;the cumulative variance contribution rate is 62.80%;the experts’ familiarity with the research(Cs)is 0.91,the authority coefficient(Cr)is 0.87,and the judgment basis coefficient(Ca)is 0.90.2.There was no significant difference between the two groups in general information,diet knowledge,attitude,practice,nutritional status,diet compliance and quality of life before intervention(p>0.05).There was no statistical significance in albumin and hemoglobin between the two groups.3.After the completion of the experiment,the albumin value of the experimental group was significantly higher than that before the intervention,with statistical significance(p<0.05);the albumin value of the control group was not statistically significant.The hemoglobin value of patients in the experimental group after intervention was significantly higher than that before intervention,with statistical significance(p<0.05);the hemoglobin value of patients in the control group before and after intervention had no statistical significance(p>0.05).The score of knowledge,attitude and practice of dietary nutrition of the main caregivers in the experimental group was statistically significant before and after the implementation of the scheme,while there was no statistical difference in the control group.The nutritional status score,quality of life score and diet compliance score of the two groups were compared,and there was significant difference in the score of the experimental group(p<0.05),while there was no significant difference in the control group.Conclusions1.The questionnaire of dietary nutrition knowledge,attitude and practice of major caregivers of MHD patients developed in this study has good reliability and validity,and can be used as a clinical evaluation tool.2.The diet plan based on Roy adaptation model is suitable for all kinds of patients,which can improve the diet compliance of patients,and improve the nutritional status and quality of life of patients. |
