Objective To explore the most suitable treatment choice by evaluating the curative effect of emergent primary resection of primary liver cancer with rupture and bleeding and secondary resection after TACE.Methods A retrospective analysis of the clinical data of60 patients treated in Wuhan Central Hospital due to rupture and bleeding of primary liver cancer from January 2008 to December 2020.These people were divided them into emergency liver cancer resection group and Transcatheter arterial chemoembolization(TACE)after the second-stage resection group based on different treatment plan.Of which36 cases were in the emergency liver cancer resection group,and 24 cases were in the second-stage resection group after interventional TACE.I made a comparison about basic conditions of the two groups,the occurrence of postoperative complications,the death in hospital,hospitalization time,hospitalization expenses,recurrence and survival.Theχ~2test and t test were used for statistical analysis,and the survival curves of the two groups of patients were drawn.Results All patients successfully stopped bleeding,and the two groups of patients had low probability of postoperative abdominal bleeding,acute liver failure,pleural effusion,ascites,bile leakage,abdominal infection,which were not statistically significant(P>0.05);there were no statistical difference on deaths within 30 days of hospitalization and tumor recurrence within 5 years between the two groups(P>0.05);There was no difference in the 3 months,6 months,1 year,2 years,and 3 years survival of the two groups of patients(P>0.05);there was a significant difference in the 4-year and5-year survival of the two groups of patients(P﹤0.05);The hospitalization cost of the hepatocellular carcinoma emergency primary resection group was significantly less than that of the combination therapy,and there was a statistical difference(P﹤0.05);the two groups had the same hospital stay,and there was no statistical difference(P>0.05).Conclusion Compared with combination treatmen,emergency primary resection of hepatocellular carcinoma can significantly save the patients’hospitalization costs and improve the patients’long-term survival.The hemostatic effect,the incidence of postoperative complications,the length of hospital stay,tumor recurrence within 5 years and the death in hospital between emergency liver cancer primary resection and TACE combined with secondary resection are comparabled.
