Objective: To explore the risk factors of lacunar infarction(LI)complicated with cerebral microbleeds(CMBs).To determine the critical value of serum uric acid(UA),fibrinogen(FIB),D-dimer(D-D)and lacunar infarction patients with cerebral microbleeds,so as to provide theoretical basis for clinical exploration of prevention,diagnosis and treatment of LI complicated with CMBs.Methods: Selected from the Department of Neurology of the affiliated Hospital of Hebei Engineering University from October 2018 to September 2020,there are 160 LI cases who met the inclusion.After being examined by plain magnetic resonance imaging and SWI,73 cases with CMBs are as CMBs group,while the other 87 cases with LI without CMBs as control group.After admission,the relevant clinical data were recorded in detail,including age,sex,smoking history,drinking history,hypertension history,diabetes history,coronary heart disease history,antiplatelet therapy history,serum fibrinogen(FIB),D-dimer(D-D)and routine biochemical indexes.Including serum uric acid(UA),cystatin C(CysC),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C).SPSS26.0 statistical software package was used to analyze the data.The risk factors of LI combined with CMBs and the critical values of UA,FIB and D-D were analyzed.Results: 1.A total of 160 patients with LI met the inclusion criteria,and 73 cases were complicated with CMBs,accounting for 45.62%,the remaining 87 cases were not associated with CMBs,accounting for 54.37%.Univariate analysis of the general data and related blood markers showed that there were significant differences in age,hypertension,diabetes,UA,FIB,D-D,TC and CysC between the two groups(P<0.05).There was no significant difference in sex,smoking,drinking,coronary heart disease,antiplatelet therapy,TG,HDL-C and LDL-C between the two groups(P>0.05).2.Multivariate Logistic regression analysis showed that age,diabetes,hypertension,high level of serum UA,high level of serum FIB,high level of serum D-D and low level of serum TC were independent risk factors for LI complicated with CMBs.3.By drawing and analyzing the ROC curve of serum UA,FIB,D-D,TC,come to a conclusion that serum UA,FIB,D-D,TC have diagnostic value for LI with CMBs(AUC>0.5,P<0.05).The Youden index of serum UA,FIB,D-D and TC were 0.29,0.36,0.35 and 0.45 respectively;the critical values of serum UA,FIB,D-D and TC were248.00umol/L,2.23g/L,0.26mg/L and 4.59mmol/L respectively;the sensitivities of serum UA,FIB,D-D and TC were 90.4%,80.8%,72.6% and 76.7% respectively;the specificities of serum UA,FIB,D-D and TC were 39.1%,55.2%,80.5% and 67.8%respectively;the areas under curves of serum UA,FIB,D-D and TC were 0.645,0.747,0.819 and 0.745 respectively.Conclusion: 1.LI with CMBs is the result of multiple factors,among which age,diabetes,hypertension,high level of serum UA,high level of serum FIB,high level of serum D-D and low level of serum TC are independent risk factors of LI complicated with CMBs.2.The critical values of serum UA,FIB and D-D were as follows:248.00umol/L,2.23g/L,0.26mg/L.They can predict the combination of LI and CMBs.
