Objective:The purpose of this study is to observe the clinical effect of cervical "partial stratification" massage combined with supine chiropractic in the treatment of cervical vertigo,and to provide a new choice for the clinical treatment of cervical vertigo.The difference between the two was compared with oral flunarizine hydrochloride capsule,and the safety,efficacy and mechanism of cervical "partial stratification" massage combined with chiropractic in the treatment of cervical vertigo were discussed.Give better play to its therapeutic advantages.At the same time,the correlation between the two evaluation indexes(vertigo intensity visual analog score and cervical vertebra dysfunction index)was analyzed to explore the pathogenesis of cervical vertigo and to provide a basis for further standardizing the diagnosis and treatment of cervical vertigo.Methods:A total of 72 patients were included in the study,6 patients dropped out,and 66 patients completed the whole study.According to the method of random number table,they were divided into a massage chiropractic group(test group,n=34)and a western medicine group(control group,n=32).The experimental group was treated with cervical "partial stratification" massage combined with supine chiropractic therapy,and the control group was treated with flunarizine hydrochloride capsule for 2 weeks.Before and after treatment,ESCV(Evaluation Scale for Cervical Vertigo),VAS(Visual Analogue Scale),NDI(Neck Disability Index)were compared,and the curative effect was evaluated.Results:1.There was no significant difference in sex,age and course of disease between the two groups(P>0.05).There was no significant difference in ESCV,VAS and NDI before treatment(P>0.05).2.Intra-group comparison: there were significant differences in ESCV,VAS and NDI scores between the two groups before and after treatment(P<0.05).3.Comparison between groups:(1)there were significant differences in ESCV,VAS and NDI scores between the two groups after treatment(P<0.05).(2)There were significant differences in the scores of ESCV,VAS and NDI between the two groups after treatment and before treatment(P<0.05).4.Comparison of clinical efficacy: the total effective rate of the test group was 94.12%,while that of the control group was 78.13%.There was no significant difference in the total effective rate between the two groups after treatment(P>0.05).There was a significant difference in the total markedly effective rate after treatment(P<0.05).The total markedly effective rate in the test group(70.59%)was higher than that in the control group(28.13%).5.The correlation between cervical dysfunction index(NDI)and visual analogue scale of vertigo intensity(VAS)showed that there was a linear correlation between the two scales(Spearman correlation = 0.362,P=0.002<0.01).Conclusions:1.Cervical "partial stratification" massage combined with supine chiropractic and oral flunarizine hydrochloride capsule can improve the symptoms of vertigo to varying degrees,but cervical "partial stratification" massage combined with supine chiropractic massage can improve the functional state of the cervical vertebra to a greater extent,which makes the relief of vertigo intensity more obvious,which is worthy of clinical promotion.2.In terms of total effective rate and total markedly effective rate,cervical "partial stratification" massage combined with supine chiropractic therapy was superior to oral flunarizine hydrochloride capsule.3.Cervical dysfunction index(NDI)and vertigo intensity visual simulation score table(VAS)have a positive correlation in the evaluation of curative effect.The side shows that the simultaneous occurrence of cervical discomfort and vertigo is not accidental,which is worthy of clinical further exploration of its potential mechanism.
