Objective Through neuropsychological evaluation,this paper explores the correlation analysis of the possible cognitive impairment caused by focal epilepsy.Methods Patients with epilepsy were collected from outpatient and inpatient hospitals of Tianjin Medical University General Hospital.Patients who met the 2017 ILEA focal epilepsy diagnostic criteria and could complete neuropsychological assessment tests.Patients without a history of progressive brain disease and brain surgery were included in the study.Subjects,a total of 53 cases.For eligible patients with epilepsy,collect relevant research data,including age,education level,age of onset of epilepsy,duration of epilepsy,form of seizures,duration of each seizure,number of AEDs,and location of EEG interval discharge.In addition,neuropsychological assessment was performed on eligible patients with epilepsy,using the Webster Intelligence Scale(<16 years old with WISC-RC,≥16 years old with WAIS-RC),Hopkins auditory language learning,Rey-Osterrieth complex graphics,d2 Attention test,connection test A and B,and the Stroop color word test were used for cognitive evaluation of intelligence,memory,language,executive function,attention and visual space.The SPSS statistical software was used to conduct a descriptive analysis of the general and clinical data of the enrolled patients,and Spearman correlation analysis of cognitive function,and then divided into two groups according to FIQ≤90 and FIQ> 90,through binary logistic regression analysis further analysis of related factors of cognition to determine whether the factors related to cognition may be a risk factor for cognitive impairment in patients with focal epilepsy.Results 1.Spearman correlation analysis showed that total IQ was related to education level,age of onset,form of onset,and total number of episodes(P <0.05),and was significantly related to AEDs and EEG interphase discharge sites(P <0.01).Memory function was related to age,education level,age of onset,number of AEDs,location of EEG discharge,total number of episodes(P <0.05).Language was correlated with education level,age of onset,number of AEDs,location of EEG discharge,and form of the attack(P <0.05);Educational level,age of onset,the number of AEDs,the location of the EEG interval discharge,and the form of the attack are related to the visual space(P <0.05).Education level,epilepsy course,seizure pattern,total number of seizures,and the number of AEDs are related to attention and executive function(P <0.05).2.Binary Logistic regression analysis showed that,of all the cognitive-related factors,only the number of AEDs(P = 0.003)and the EEG interval discharge(P = 0.013)are the risks factors of cognitive impairment in patients with focal epilepsy.Conclusions 1.The education level,age of onset,duration of seizures,seizure pattern,total number of seizures,number of AEDs,and EEG intermittent discharge sites in patients with focal epilepsy may be related factors of cognitive function.Intelligence is a comprehensive assessment of cognitive function.The relevant clinical factors of intelligence include education level,age of onset,number of AEDs,location of EEG discharge,form of attack,and total number of attacks.2.Among the factors related to cognitive function in this study,The number of AEDs and the site of the EEG discharge may be risk factors for cognitive impairment in patients with focal epilepsy.3.In the clinical treatment of epilepsy,we must actively control seizures and reduce brain damage caused by seizures.Drug treatment is the most common treatment method.For drug treatment,starting from a single drug,surgical treatment or other non-surgical treatment(eg ketogenic treatment)can be considered when there are more than 3 drugs.At the same time,we should also pay attention to the changes of EEG in the onset of interphase,and regularly review the EEG in the inter-period.
