Objective:To investigate the risk factors of H-type hypertension(HHT)combined with acute ischemic stroke(AIS),and further discuss the characteristics of cognitive function of patients with HHT and AIS,then analyze the correlation between related factors and cognitive function.Materials and methods:A total of 127 consecutive inpatients with hypertension and AIS from the neurology department in Hebei General Hospital between March 2018 and September 2018 were selected in our study.According to the plasmatic homocysteine(Hcy)levels patients were divided into two groups:(HHT group:Hcy≥10umol/L 73 cases,none HHT group:Hcy<10ummol/L 54 cases).The general data,blood biochemistry and radiological data were collected.The mini-mental state examination(MMSE)and Montreal cognitive assessment(MoCA)were used to assess cognitive function in the two groups.Logistic regression analysis,χ~2 test,T-test were used to analyze the related risk factors of HHT combined with AIS.Then T-test andχ~2 test were used to compare the difference of cognitive function between the two groups,and the relationship between the scores of MoCA and related factors in the HHT group was calculated by Pearson correlation.Results:1.The HHT group included a higher prevalence of EPVS and living in rural areas compared with the none HHT group(P<0.05).The level of BMI,BUA,TG and FBG were higher in HHT group than none HHT group(P<0.05).2.The multivariate logistic regression analysis showed that the elevated level of BUA(OR=2.040,95%CI:1.262~3.232),TG(OR=1.967,95%CI:1.095~3.001)and FBG(OR=1.725,95%CI:1.011~3.030)were all the independent risk factors for HHT combined with AIS.3.The proportion of cognitive impairment in HHT group was higher than that in none HHT group(72.6%vs 55.6%,P<0.05),and the total scores of MMSE and MoCA in HHT group were lower than those in none HHT group(P<0.05).4.Pearson correlation analysis showed that the total score of MoCA in HHT group was negatively correlated with the levels of Hcy(r=-0.656,P=0.002)and BUA(r=-0.553,P=0.032).The level of Hcy was negatively correlated with the scores of visual space and executive performance,attention and calculation,language,abstract thinking and delayed memory.While BUA level was negatively correlated with the scores of visual space and executive performance,naming,attention and calculation,language,abstract thinking and delayed memory.Conclusion:1.Patients with HHT and AIS are more common in rural residents.Their BMI levels are higher and EPVS are more likely to occur.2.The elevated level of BUA,TG and FBG were all the independent risk factors for HHT combined with AIS.3.Patients with HHT and AIS may be more prone to be cognitive impairment.The degree of cognitive decline is closely related to the level of Hcy and BUA.
