Objective 1.To understand the current status of self-management behaviors of patients with hypertensive comorbidities through interviews.2.Through cross-sectional research,under the guidance of the theory of health belief model,explore the influencing factors of self-management behaviors of hypertensive patients with co-morbidities.The characteristic changes of influencing factors of self-management behavior in patients with hypertension were described from the aspects of disease susceptibility,severity,perceived benefit,perceived disorder,behavioral cues and self-efficacy.Methods 1.Use interviews to investigate the self-management experience and life experience of 10 hypertensive patients with co-morbidity,explore the influencing factors of self-management behavior of hypertensive patients with co-morbidity,so as to improve the level of self-management for patients,and develop nursing intervention measures for nursing staff,Provide a basis for improving the prognosis of chronic diseases.2.A cross-sectional study method was used to select 200 outpatient and hypertensive comorbid patients who were treated in a secondary hospital in Tianjin from November 2019 to December 2019,and surveyed the patients’ age,gender,and education level through questionnaires.Comorbidities,the severity and susceptibility of the disease,the benefits of behavioral changes and cognitive impairments,clues to actions,self-efficacy,and other health beliefs affect the self-management behavior of hypertensive patients with comorbidities.Univariate analysis,multivariate analysis and correlation analysis were used to investigate the factors affecting the self-management behavior of patients with hypertension.Results 1.Interviews with 10 patients with hypertension and co-morbidity: the patient’s lack of self-management awareness in the early stage of the disease,the patient’s age,education,and severity of the co-morbidity will affect their self-management behavior;With the prolonged illness,patients gradually realize the importance of self-management,the benefits of self-management and the obstacles encountered affect the formation of self-management behaviors;and family,medical care,social and other supporting factors and the self-efficacy of patients are patients In the future life,we can insist on the main factors of self-management of diseases.2.Through questionnaire survey on 200 hypertensive comorbid patients,the following results were obtained: 2.1 The total score of patients with hypertension comorbidity is(96.43±15.11)points.Compared with the survey results of Zeng Yongjun on ordinary people,the total score of patients with hypertension comorbidity is lower than that of ordinary people(P<0.05).Severity,perceived obstacles,action cues,and self-efficacy were not significantly different from those of ordinary people(P>0.05),and the score of perceived benefits was lower than that of ordinary people(P<0.05).2.2 Using self-management score as the dependent variable,a single factor analysis was conducted based on gender,education level,marital status,family monthly income per capita,payment method of medical expenses,occupation,and place of residence.The results showed different levels of education,occupation,hypertension course,whether or not The difference in self-management behavior of patients who received health education was statistically significant(P<0.001).2.3 Taking self-management behavior as the dependent variable,correlation analysis was carried out on the six dimensions of the number of comorbidities,the number of hospitalizations per year,the health belief,and the cumulative score of disease.There was a negative correlation between the cumulative scores of disorders and diseases(P<0.001);positive correlation with susceptibility,severity,perceived benefits,and self-efficacy in health beliefs(P<0.001).2.4 Taking the self-management score as the dependent variable,the variables with single-factor analysis results were statistically significant,including the course of hypertension,whether they received health education,education level,occupation,number of hospitalizations per year,self-efficacy,cumulative disease score and health The six dimensions of belief are independent variables for multiple linear regression analysis,The results showed that the five items of self-efficacy,perceived obstacles,severity,susceptibility,action cues,and cumulative disease score in health beliefs are independent influencing factors of self-management behaviors in hypertensive patients(P<0.05).Conclusion 1.Through interviews with patients with hypertensive comorbidities,it is concluded that the self-management consciousness of patients with hypertensive comorbidities needs to be strengthened in the early stage of the disease to improve the patient’s disease management level;with the formation of self-management consciousness,assist patients to recognize themselves Benefits of management and overcoming obstacles encountered,patients will gradually establish self-management behaviors;enhance the role of support systems and patient self-efficacy,consolidate patient self-management behaviors,and ultimately help patients establish long-term self-management behaviors.2.Through a cross-sectional survey of 200 hypertensive patients with co-morbidity,it was concluded that the self-management behavior scores and health conviction scores of hypertensive patients with co-morbidity were lower,and the cumulative condition of the disease was more serious.Different education levels,occupations,and course of hypertension The number of hospitalizations per year,whether you have received health education,and health beliefs will affect the self-management behavior of patients with hypertension and co-morbidity;we must strengthen the health beliefs of patients’ self-efficacy,perceived obstacles,severity,susceptibility,action clues,etc.Education and guidance to improve patient self-management behavior.
