| Objective: In this study,through applying modified linggui fuping decoction combined with acupuncture and moxibustion,we observe the clinical efficacy and safety of treatment of hepatitis B cirrhosis patients with refractory ascites(cold-dampness perplexing spleen type),then evaluate the advantages of combining traditional Chinese with western medicine treatment of the disease and discusses optimization scheme.For the treatment of hepatitis B cirrhosis patients with refractory ascites to provide scientific and effective safety guidelines,promoting the development of internal and external governance of traditional Chinese medicine.Methods:By using randomized,controlled clinical research methods,a total of 78 patients with refractory ascites of hepatitis B cirrhosis(cold-dampness perplexing spleen type)who met the inclusion criteria were selected and divided into treatment group and control group by random number table method at 1:1.The control group was given conventional treatment in western medicine,and the treatment group was treated with modified linggui fuping decoction combined with acupuncture and moxibustion on the basis of conventional treatment in western medicine.4 days for a course,three consecutive period of treatment.The main symptoms,signs,liver function and the depth of ascites under color ultrasound were observed and recorded in the two groups of patients before and treatment.Body weight,abdominal circumference and 24-hour urine volume of the patients were observed and recorded before and after each course of treatment.The efficacy of modified linggui fuping decoction combined with acupuncture and moxibustion were evaluated;The safety of adverse reactions before and after treatment also been evaluated.Results :(1)Comprehensive efficacy: the total effective rate was 92.11% in the treatment group and 71.79% in the control group,and the treatment group was superior to the control group(P<0.05).(2)Comparison of TCM symptom efficacy: after the course of treatment,the total effective rate of TCM symptoms was 97.37% in the treatment group and 74.36% in the control group.The treatment group was better than the control group,and the difference was statistically significant(P<0.05).The TCM symptoms of both groups improved after treatment.The treatment group was superior to the control group in terms of abdominal fullness,abdominal distension pain,fatigue and loose stool on the basis of internal and external treatment of traditional Chinese medicine(P<0.05),and there was no significant difference between the two groups in the edema of lower limbs(P>0.05).(3)Comparison of liver function: ALT,AST,TBIL and ALB all improved after treatment in the two groups(P<0.05).There were statistically significant differences in ALT,AST and TBIL in the treatment group compared with the control group(P < 0.05),and there was no significant difference in ALB improvement between the treatment group and the control group(P>0.05).(4)The comparison of lower abdominal water volume by color doppler ultrasound between the two groups improved after treatment(P<0.05).The depth of ascites in the treatment group was significantly different from that in the control group(P<0.05),and the treatment group was superior to the control group.(5)Body weight,abdominal circumference and urine volume at 24h: Both groups showed improvement in reducing abdominal circumference,weight and increasing urine volume.(1)Time factor,weight,abdominal circumference and 24 h urine volume at different times of the drug efficacy are different(P<0.001).(2)There was an interaction effect between the treatment body weight and abdominal circumference of the two groups in the time * group(P<0.05),which was significantly affected by the treatment group with the change of time.(3)In the analysis of variance of effect between groups,there were significant differences in body weight and 24 h urine volume among different treatment methods(P<0.05),and the treatment group was better than the control group.(4)At the same time point between the two groups,compared with the control group,the treatment group weight and abdominal circumference at the 8th and 12 th day of treatment had significant statistical differences(P<0.05);There were statistically significant differences in urine volume at24h in the treatment group at day 4,8 and 12(P<0.05),the effect of urine volume treatment was earlier than body weight and abdominal circumference.Conclusion: Modified linggui fuping decoction combined with acupuncture and moxibustion has significant effects on improving the clinical symptoms,restoring liver function in patients with refractory ascites due to hepatitis B cirrhosis(cold-dampness perplexing spleen type).Through the repeated measurement of a short course of treatment,the modified linggui fuping decoction combined with acupuncture and moxibustion can rapidly increase the urine volume,reduce the patient’s weight,reduce abdominal circumference,and promote the regression of ascites in patients with refractory ascites due to hepatitis B cirrhosis.In the treatment process,due to the safe use of the prescription,the operation of acupuncture and moxibustion technology,the curative effect is stable and the safety is high,which provides a reference for the clinical treatment of refractory ascites to choose the optimal plan. |
