Objective:To explore the specificity of brain functional activity in patients with lumbar disc herniation radicular pain;and the effect of Huaisanzhen on brain functional activity in patients,and to explore new ways for the study of acupuncture analgesic central mechanism.Methods:Twenty patients with lumbar disc herniation radicular pain and twenty healthy volunteers who met the inclusion criteria were divided into the radicular pain group and the healthy control group.The radicular pain group was treated with one-time Huaisanzhen treatment.Before and after treatment,the VAS and straight leg elevation angle were used as the evaluation indicators,and the resting state f MRI scans were performed.The healthy control group was scanned the resting state f MRI only once as a control.The Amplitude of Low Frequency Fluctuations(ALFF)analysis method was used to process imaging data.Results:1.There was no statistical significance between the two groups of subjects in terms of age,gender,and education level(P>0.05),suggesting comparability.2.Compared with before treatment,the VAS score of the radicular pain group was significantly reduced after acupuncture treatment,and the straight leg elevation angle was significantly increased.The difference between the two was statistically significant(P<0.05).3.Compared with the healthy control group,the brain regions with abnormal brain function activity in the radicular pain group include: left superior occipital gyrus,middle occipital gyrus,inferior occipital gyrus,lingual gyrus,fusiform gyrus,precentral gyrus,right calcarine fissure cortex,precuneus,posterior cingulate cortex,etc.The ALFF value of the brain regions were significantly increased(P<0.05);left middle temporal gyrus,inferior temporal gyrus,etc.The ALFF value of the brain regions were significantly reduced(P<0.05).4.Compared with before treatment,the brain regions with abnormal brain function activity after acupuncture treatment in the radicular pain group include: left inferior cerebellum,etc.The ALFF value of the brain regions were significantly increased(P<0.05);left superior parietal gyrus,precuneus,etc.The ALFF value of the brain regions were significantly reduced(P<0.05).Conclusion:1.Huaisanzhen can significantly reduce the pain of patients with lumbar disc herniation radicular pain,improve functional activity of lower limbs,and have a significant clinical effect.2.The brain regions with abnormal brain functional activity in patients with lumbar disc herniation radicular pain are mainly in the occipital lobe,temporal lobe,precentral gyrus,precuneus,posterior cingulate cortex,etc.This abnormal change may be one of the pathological changes of central functional activity characteristics in patients with lumbar disc herniation.3.The clinical effects of Huaisanzhen may be reflected by changes in the functional activities of the cerebellum,parietal lobe,and precuneus,etc.
