Objective:The purpose of this article is to provide a more reasonable surgical solution for the surgical treatment of early non-small cell lung cancer by comparing the efficacy of patients with early non-small cell lung cancer after segmentectomy or lobectomy.Methods:From March 2016 to March 2017,we screened the patients who were hospitalized in the Affiliated Hospital of Binzhou Medical University,found the lung masses by computed tomography and the masses were limited to a single lung segment,and were confirmed as non-small cell lung cancer by pathology after segmental resection or lobectomy in the Affiliated Hospital of Binzhou Medical College.The surgical data,postoperative complications,long-term recurrence rate and survival rate were compared between the two groups.Results:A total of 270 patients were selected,including segmental resection group(S group)128 cases,lobectomy group(L group)142 cases.1.Operation data between the two groups:operation time(153.96±19.15)min,intraoperative bleeding volume(145.37±31.41)ml,drainage tube removal time(2.63±0.63)d,postoperative hospital stay time(3.92±1.18)d in group S.The difference of operation time(131.44±22.52)min,intraoperative bleeding volume(189.17±37.9)ml,drainage tube removal time(3.92±1.29)d and postoperative hospital stay time(5.2±1.17)d,was statistically significant(P<0.05).There was no significant difference in the number of lymph node dissections between group S(16.17±4.55)and group L(17.37±6.72)(P>0.05).2.Postoperative complications:There were 1 case of atelectasis,2 cases of pulmonary infection,0 case of arrhythmia and 1 case of alveolar leakage in group S,4 cases in total.There were 5 cases of atelectasis,6 cases of pulmonary infection,6 cases of arrhythmia and 1 cases of alveolar leakage in group L,a total of 18 cases.Compared with the data between the two groups,the number of complications in group S was less than that in group L,and the difference between the two groups was statistically significant(P<0.05).3.The recurrence rate and survival rate were compared between the two groups:3 cases in group S were followed up 3 years after operation,no death.In group L,2 cases recurred 3 years later,and 1 case died.But there was no significant difference in recurrence rate and survival rate between the two groups(P>0.05).Conclusion:For patients with early non-small cell lung cancer,segmental resection and lobectomy can be used as surgical treatment.Compared with lobectomy,segmental resection has the advantages of small resection range,rapid extubation,short hospital stay,less postoperative complications,safety and clinical application value.
