Discussion On Professor Caiyun Zhou’s Experience In Treating Behcet’s Disease Based On Bibliometrics Analysis And Data Mining

1.Research BackgroundBehcet disease(BD)is an autoimmune-related vascular inflammatory disease characterized by oral and vulvar ulcers,ophthalmitis and skin damage.The disease can involve multiple systems in its late development.The etiology and pathogenesis of Behcet’s disease are still unclear.Western medicine does not have specific drugs for the treatment of Behcet’s disease for the time being,and it mainly starts with local treatment and symptomatic treatment.However,most western medicines have large side effects and are prone to relapse after drug withdrawal.For the treatment of this disease,the Chinese medicine from the treatment of the root,syndrome differentiation treatment,for Behcet’s disease treatment has certain advantages,in the treatment of the disease at the same time,can also alleviate the use of western medicine brought about by the toxic side effects.The treatment of Behcet’s disease by traditional Chinese medicine has a long history,and it has been continuously innovated in the process of inheritance.However,there is no bibliometrics analysis on the system of treating Behcet’s disease by traditional Chinese medicine at present.To summarize the development and status quo of treating Behcet’s disease by traditional Chinese medicine,understanding the development and status quo of treating Behcet’s disease by traditional Chinese medicine is of great significance for the development and innovation of treating Behcet’s disease by traditional Chinese medicine in the future.Therefore,this paper analyzes the literature on the treatment of Behcet’s disease by traditional Chinese medicine in the past 20 years,so as to provide clinical and scientific reference for the treatment of Behcet’s disease in the future.At the same time,data mining method was used to explore Professor Zhou Caiyun’s experience in treating Behcet’s disease,and to summarize the way of drug use and the rule of differentiation and treatment.2.Research ObjectivesThis paper aims to summarize the medication rules of traditional Chinese medicine in the treatment of Behcet’s disease through bibliometrics analysis of literatures in the recent 20 years(2000-2020),and provide clinical and scientific reference for the treatment of Behcet’s disease by traditional Chinese medicine.It also conducts data mining on Professor Zhou Caiyun’s experience in the clinical treatment of Behcet’s disease,discusses and summarizes Professor Zhou Caiyun’s experience in the clinical treatment of Behcet’s disease,and explores the thoughts of syndrome differentiation and medication rules of Dr.Zhou Caiyun’s in the treatment of Behcet’s disease.3.Research method3.1 Bibliometrical research methodsThe CNKI database was used to search relevant literatures on TCM treatment of Behcet’s disease.The literatures that met the inclusion criteria were screened by reading,Excel and CiteSpace were used to conduct statistical analysis on the contents of the literatures(publication time,published journal,core author,literature type,TCM syndrome type and TCM treatment),and the development,diagnosis and treatment rules of TCM treatment of Behcet’s disease were summarized and summarized.3.2 Data mining research methodsUsing the Chinese medicine inheritance auxiliary platform(V2.5),the patients with Behcet’s disease diagnosed and treated by Chief Physician Zhou Caiyun’s from January 2018 to January 2021 in the Rheumatology Department of Xi Yuan Hospital and inpatients,a total of 80 people,80 clinical cases of data that were diagnosed and treated by the instructor herself,and follow-up records of the follow-up members.The content includes the patient’s general condition,medical history,symptoms,TCM diagnosis and treatment methods,tongue and pulse information,prescriptions,etc.Using descriptive statistics,association rules,clustering analysis and data mining methods to excavate the basic situation,the distribution of disease syndrome types,the frequency of medication and the law of drug compatibility of patients with Behcet’s disease diagnosed and treated by Chief Physician Zhou Caiyun’s.The academic experience of chief physician Zhou Caiyun’s diagnosis and treatment of Behcet’s disease was summarized by man-machine combination.4.Research results4.1 literature metrology analysis results4.1.1 Basic characteristics of literatureA total of 330 literatures were included in this study,including 78 famous doctors’ experiences,35 case reports,115 clinical studies,6 experimental studies,64 theoretical studies,27 review studies and 5 literature studies.Although the annual number of published articles fluctuated,it showed an overall trend of growth,with two obvious peaks in 2008 and 2010 respectively.According to the analysis of the distribution of literatures in the regions where the first author worked,330 literatures were distributed in 22 provinces,4 autonomous regions and 4 municipalities directly under the central government(excluding Hong Kong,Macao and Taiwan),among which the top 10 regions were Shandong,Beijing,Jiangsu,Zhejiang,Henan,Heilongjiang,Guangdong,Gansu and Ningxia Hui Autonomous Region,Shanghai and Liaoning.The included 330 literatures were distributed in 119 journals,and the top 10 literatures were New Traditional Chinese Medicine,Journal of Traditional Chinese Medicine,Sichuan Traditional Chinese Medicine,Hebei Traditional Chinese Medicine,Rheumatism and Arthritis,Shandong Journal of Traditional Chinese Medicine,Traditional Chinese Medicine Research,Chinese Folk Therapy,Modern Journal of Integrated Traditional Chinese and Western Medicine,and Shaanxi Traditional Chinese Medicine.4.1.2 The characteristics of TCM syndrome differentiation and medicationThere were 185 syndromes in total,74 different syndromes.The key words of 74 different syndromes were extracted and classified.The frequency of occurrence from high to low was followed by damp-heat,Yin deficiency,heat toxicity,spleen deficiency,blood stasis,Yang deficiency,qi deficiency,liver stagnation,qi stagnation,hyperactive heart fire,blood deficiency and blood heat.Therapeutic methods of Behcet’s disease are varied,among which 234 literatures involving internal treatment,9 literatures involving external treatment and 60 literatures involving internal treatment combined with external treatment.The internal decoction for Behcet’s disease can be divided into specific prescriptions and tested prescriptions.Among the 10 prescriptions with the highest frequency of use in the acute stage,the most frequently used prescriptions licorice xiexin decoction,longdan xiegan decoction,si miao yong an decoction,rhino jiao dihuang decoction,zhi bai di huang decoction,wu wei disinfection decoction,qing wei powder,red adousia dan dang gui powder,dao chi powder and rhizoma rhizoma detoxifying decoction.The 10 herbs with the highest frequency of use in the specific prescriptions are licorice,phellophellae phellophellae,radix scutellariae,rhizoma rhizoma,rehmannia glutinosa,angelica sinensis,soil poria coco,honeysuicle,peony bark and red peony root.The prescription used more frequently than twice in the remission period were Buzhong Yiqi Decoction,Jinkui Shenqi Pillar,Liuwei Dihuang Pillar,Sijunzi Decoction,Huangqi Guizhi Wuwu Decoction,Shenling Baizhu Powder,Chidou Dou Danggui Powder.The ten traditional Chinese medicines used in the specific prescription with the highest frequency were:poria,white paeonia lactiflora,Atractylodes Radix Atractylodes,Angelica Angelica,Dangshen Radix Radix,Astragalus membranus,Rehmannia glutinosa,Rehmannia glutinosa,Phelhelus chinensis,Radix Ophiopogonis;Among the external treatment methods,the most frequently used traditional Chinese medicines of fumigated external washing prescription were:Sophora flavescens,Phellodendron phellodendron,Fructus Cnidii,Fructus dactyloides,Fructus dactyloides,Borneol,Coptis coptis,Radix Alba,etc.;Acupoints with higher frequency of acupuncture and moxibustion selection are:Hegu,Ganshu,Zusanli,Sanyinjiao,Xinshu and Pishu.4.2 Data mining research results4.2.1 General information of patientsAmong the 80 medical cases treated and treated by Professor Zhou Caiyun,36 cases were male and 44 cases were female,with a male-to-female ratio of 1:1.22.Most of the cases were between 20 and 60 years old,accounting for 83.8%of the total cases.4.2.2 Basic symptoms of patientsThe main symptoms of patients with Behcet’s disease included were oral ulcer(82.1%),arthralgia(27.4%),dyspnea(23.8%),genital ulcer(22.6%),dysphoria(15.5%),erythema nodosus(15.5%),sore throat(14.3%),facial acne(10.7%),etc.The main tongue signs were red tongue(60.0%),light red tongue(26.35%),dark tongue(17.5%)and toothed tongue(13.8%).The main coating on the tongue were yellow and greasy(37.5%),white(31.3%),white and greasy(12.5%),thin white(12.5%),thin yellow(7.5%),etc.The main pulse characteristics were pulse fineness(42.5%),pulse smoothness(27.2%),pulse chord(10.0%),pulse smoothness number(10.0%),pulse fineness(10.0%),pulse number(10.0%)and so on.4.2.3 Syndrome differentiation and medication ruleThe main syndrome differentiation types of Behcet disease patients included included dampness-heat accumulation(61.3%),dampness-sleepiness of spleen deficiency(13.8%),Qi and Yin deficiency(8.8%),dampness-heat of Yin deficiency(6.3%),dampness-heat of liver and gallbladder(5.0%),and heat-toxic accumulation(3.8%),etc.The main treatment methods include clearing heat and reducing dampness(67.5%),invigorating spleen and reducing dampness(12.5%),nourishing qi and nourishing Yin(8.8%),nourishing Yin and clearing heat(7.5%),clearing heat and detoxification(6.3%),clearing liver and reducing fire(6.3%),etc.Higher frequency of usage of drugs for honeysuckle(90.5%),licorice(86.9%),radix scrophulariae(84.5%),poria cocos(78.6%),angelica(72.6%),radix paeoniae rubra(65.5%),fructus arctii(63.1%),semen coicis(61.9%),leaf(50.0%),Thunberg Fritillaria(47.6%),phragmites communis(45.2%),cogongrass rhizome(42.9%),lily(41.7%),and dandelion(39.3%),rhizoma coptidis(31.0%),etc.;Based on association rules,it is concluded that the core drug 8,respectively,flos lonicerae,radix scrophulariae,radix paeoniae rubra,angelica,licorice,fructus arctii,poria cocos,coix seed,and 65 high frequency medicine group,the high frequency of medicine group respectively honeysuckle-figwort root,honeysuckle,radix glycyrrhizae,radix scrophulariae,radix glycyrrhizae,flos lonicerae,radix scrophulariae,radix glycyrrhizae,radix scrophulariae,angelica,etc.;Analysis method based on entropy formula law 16 core drug combinations,respectively for butterfly,boat-fruited sterculia,wood,radix rhizoma imperatae,corrugated,folium isatidis,cogongrass rhizome,corrugated,radix paeoniae rubra,main licorice,licorice,radix scrophulariae,dandelion,turmeric,zizyphus jujube kernel,etc.,and eight New Zealand combination,radix scutellariae,boat-fruited sterculia respectively,wood butterfly,big belly skin,cogongrass rhizome,corrugated,folium isatidis,cuttlebone,cogongrass rhizome,corrugated,radix paeoniae rubra,cuttlefish bone,etc.5.Research ConclusionThrough the quantitative analysis of 330 literatures,it is found that the onset of Behcet’s disease is closely related to the causes of dampness toxin and spleen deficiency.Through the data extraction of clinical efficacy medical cases of Behcet’s disease treated by Chief Physician Zhou Caiyun,In terms of clinical medication,Chief Physician Zhou Caiyun uses Simiao Yongan as the basis for the treatment of syndrome differentiation.
