Purpose:through comprehensive AECOPD and OP of Chinese and western medicine research results,the design of clinical research program,exploreAECOPD of the correlation between TCM syndrome type with bone mineral density,for AECOPD provided a reference for clinical treatment and basic researchoftraditional Chinesemedicineand for prevention and treatment of OP clinical guidance.Methods:A total of 104 patients with AECOPD who were admitted to the Department of Traditional Chinese Medicine of the Affiliated Hospital of North Sichuan Medical University from January 2018 to June 2020 were collected according to the inclusion criteria and exclusion criteria.Record patients’ gender,age,course of general data,such as determination of lung function index,dual-energy X-ray absorptiometry(dexa),"the determination of 1,2,3,4 patients with lumbar spine and neck,Wards area,large tuberosity T value,the statistics of chronic obstructive pulmonary disease in patients with acute aggravating period rate of bone loss,osteoporosis,with reference to China press of traditional Chinese medicine journal of internal medicine of traditional Chinese medicine(eighth edition)[1]on lung expansion is discussed;Li Jiansheng etal.[2]published "Establishment of Syndrome Diagnostic Criterion of Chronic Obstructive Pulmonary Disease in the Acute Agglomeration Stage" after clinical investigation and study of TCM syndrom of AECOPD by using multi-center and prospective research method,and divided patients into four syndrom types:Qi stagnation and blood stasis,wind-cold obstruction of lung,phlegm-heat obstruction of lung,and phlegm-turbidities obstruction of lung.The correlation between different syndromes and T value was investigated by GraphPad Prisma correlation analysis.Results:In this study,the detection rate of bone loss in patients with acute exacerbation of COPD was 33%,and the detection rate of osteoporosis was 34%.Among them,the detection rate of osteoporosis in AECOPD type with qi stagnation and blood stasis was significantly higher than that in AECOPD type with wind-cold obstruction of lung,phlegm-heat obstruction lung and phlegm-turbidity obstruction lung,which was as high as 50%,and the comparison with the three groups was statistically significant.There was a significant correlation between symptom scores and BMD in AECOPD patients(P<0.01),and there was a negative correlation between symptom scores and BMD in the Qi stagnation and blood stasis group,while there was no statistical significance in the other three groups(P>0.05).The degree of airflow limitation was also significantly correlated with bone mineral density in AECOPD patients,among which the degree of airflow limitation was negatively correlated with bone mineral density in the Qi stagnation and blood stasis group,while there was no statistical significance in the other three groups(P>0.05).In addition,there was no correlation between hypercapnia,hypoxemia,hospitalization times and long-term use of glucocorticoids(P>0.05).Conclusion:Chronic obstructive pulmonary diseasewith acute aggravating period of the TCM syndrome type osteoporosis detection rate,bone loss detection rate were higher than the general population,and the degree of bone loss and airflow limited degree,syndrome integral as a significant correlation,among which the osteoporosis detection rate,qi and blood stasis syndrome integral,limited airflow level were higher than the other three groups,and has significant difference(P<0.01),suggesting we osteopenia perforative and whole process of chronic obstructive pulmonary disease,in acute aggravating period take positive,is very important for the treatment and prevention science,at the same time when the disease development to the middle and later stage,Also should actively prevent pathological bone and trauma fracture,avoid fracture aggravation condition. |
