ObjectiveTo construct a tool for investigating the dampness syndrome of membranous nephropathy to assess the severity of dampness syndrome in patients with membranous nephropathy,to explore the correlation between urine protein and dampness syndrome in patients with membranous nephropathy,and to provide valuable information for the clinical application of the method of removing dampness to treat membranous nephropathy.MethodsOn the basis of the "Dampness Syndrome Syndrome" in the "Syndrome Dialectics",the relevant literature was searched,and the membranous nephropathy damp syndrome questionnaire was formed after expanding the wet syndrome related symptoms.Membranous nephropathy patients admitted to the membranous nephropathy clinic and nephrology department will conduct pre-test questionnaire analysis to form a wet syndrome survey tool with good validity and reliability for this population.According to the latest urine protein test(24h urine protein or urine protein-creatinine ratio)when the patients were enrolled,all included patients were divided into low urine protein group(proteinuria less than 3.5g)and moderate urine protein group(greater than or equal to 3.5g and less than or equal to 8g)and high urine protein group(greater than 8g),use the damp syndrome survey tool to score all patients,and perform the Man-Whitney rank sum test between the groups of each item and the total score of each group and use the ROC curve method to explore the diagnostic performance of this damp syndrome investigation tool in patients with membranous nephropathy and high urine protein to explore the correlation between urine protein and the severity of dampness syndrome in this population.Results1.On the basis of "Dialectics of Syndrome",search and carefully read 72 related documents,and the preliminary results are caused by edema,obesity,sore and heavy limbs and joints,weakness or fatigue,mental fatigue or lethargy,and dirty complexion if there is any Dirt,oily face or hair,sticky mouth,heavy breath,thirsty but do not want to drink water,poor body heat,poor sweating,nausea or nausea and vomiting,full abdominal abdomen,cloudy urine or obvious foamy urine,stool Irregularities,frequent bowel movements,sticky stools,covered head,poor appetite,excessive volume/scrotum moist,skin sores or dampness,thick tongue coating,greasy tongue coating,slippery and fluid tongue,fat tongue A questionnaire for membranous nephropathy damp syndrome consisting of 26 items.2.A total of 52 patient questionnaires were collected in the study.After the analysis of the pre-test questionnaire,the results were composed of edema,joint pain and heavy limbs,weakness or fatigue,mental fatigue or lethargy,dirty complexion such as dirt,sticky mouth,nausea or nausea Vomiting,full abdominal distension,turbid urine or obvious foamy urine,poor appetite,excessive entrapment/scrotal dampness,a membranous nephropathy survey tool consisting of 11 items.3.After scoring the low,medium,and high urine protein groups using the damp syndrome investigation tool,the results of the Mann-Whitney rank sum test of the scores and total scores of each item among the three groups:(1)The symptom scores of edema,sticky mouth,turbid urine,or obvious foamy urine and the total score of dampness syndrome in the low proteinuria group were lower than those in the medium proteinuria group and were statistically different(P<0.05).(2)The symptom scores of edema,limb weakness or fatigue,full abdominal distension,turbid urine or obvious foamy urine,poor appetite and total scores of damp syndrome in the low proteinuria group were lower than those in the high proteinuria group and were statistically different(P<0.05).(3)The symptom scores of poor appetite and the total score of wet syndrome in the middle urine protein group were lower than those in the high proteinuria group and were statistically different(P<0.05).4.The ROC curve indicates that this damp syndrome investigation tool has good diagnostic performance for patients with membranous nephropathy with high urine protein(urinary protein>8g),and the area under the ROC curve is 0.914(P<0.05).The critical value of wet syndrome for patients with high proteinuria in this population is 22.5 points.ConclusionThe degree of urine protein in patients with membranous nephropathy is related to the dampness syndrome.The more severe the dampness syndrome,the more obvious the symptoms and the more urine protein.When the symptoms of dampness syndrome reach a certain level,it has a certain predictive effect on the appearance of high urine protein(urine protein>8g/d)in patients with membranous nephropathy.When using traditional Chinese medicine in the treatment of membranous nephropathy,the method of removing dampness can be rationally used according to the patient’s symptoms.For patients with high urine protein(urinary protein>8g/d),more attention may be paid to removing dampness and strengthening spleen. |
