Study On Traditional Chinese Medicine Treatment Of Ulcerative Colitis Based On Literature And Case Review

ObjectiveThrough literature evidence-based medicine research and clinical retrospective study,this paper summarized the present situation of TCM treatment of ulcerative colitis,analyzed the composition and distribution of TCM treatment,and discussed the influencing factors of TCM treatment selection.In order to provide useful reference for the choice of TCM treatment and the formulation of standardized and popularized treatment schemes of TCM treatment.Methods1.Database Literature Retrieval:Search CNKI,Wanfang,VIP,PubMed,Cochrane Library and Embase on the treatment of ulcerative colitis with TCM from January 1,2011 to December 31,2020.Finally,134 articles were included,and the data of literature title,research type,sex,age,course of disease,TCM treatment,prescription and other materials were extracted.2.Clinical data collection:Collect the medical records of patients diagnosed with ulcerative colitis in the outpatient system of Guangdong Province Hospital of Traditional Chinese Medicine from January 1,2011 to December 31,2020.Finally,162 cases were included,and the data of gender,age,course of disease,clinical type,stage of disease,severity of disease,TCM syndrome scores,TCM treatment,prescriptions and other materials were extracted.3.Study the composition and distribution of the main treatment of database literature and clinical data,summarize the current situation of TCM treatment of ulcerative colitis.In the clinical data,Univariate analysis was made on the factors affecting the choice of TCM treatment,such as sex,age,course of disease,clinical type,total score of main symptoms,total score of secondary symptoms,tongue,tongue coating,pulse.According to the results,the factors with statistical significance and statistically insignificant but clinically thought to affect the choice of treatment were included in multi-classification Logistic regression analysis to analyze the influencing factors of TCM treatment choice.Results1.The constituent ratios of TCM therapies in the literature were as follows:clearing heat and removing dampness(41.41%),invigorating spleen and replenishing qi(20.08%),other therapies(13.96%),warming and tonifying spleen and kidney(12.60%),soothing liver and invigorating spleen(6.81%)and mildly regulating cold and heat(5.16%).2.The constituent ratios of TCM therapies in the clinical data were as follows:invigorating spleen and removing dampnes(32.10%),clearing heat and removing dampness(22.22%),mildly regulating cold and heat(14.20%),other therapies(12.96%),invigorating spleen and replenishing qi(11.73%),soothing liver and invigorating spleen(3.09%),nourishing yin and blood(2.47%),warming and tonifying spleen and kidney(1.23%).3.The tongue,tongue coating,pulse with statistical differences in univariate analysis and the gender,age,course of disease,clinical type,total score of main symptoms,total score of secondary symptoms which were considered to be closely related to TCM treatment,were included in the multi-classification logistic regression analysis.The final analysis showed that the factors of age,course of disease,clinical type,total score of secondary symptoms,tongue,tongue coating and pulse can influence the choice of TCM treatment(P<0.05).ConclusionIn the TCM treatment of UC,among the literature reports,that clearing heat and dampness,invigorating spleen and Qi are the most commonly used,followed by warming and tonifying spleen and kidney,soothing liver and invigorating spleen,and mildly regulating cold and heat.Among the clinical data,that invigorating spleen and removing dampnes,clearing heat and dampness,mildly regulating cold and heat are the most commonly used,followed by invigorating spleen and Qi,soothing liver and invigorating spleen,nourishing yin and blood,warming and tonifying spleen and kidney.Invigorating spleen and removing dampnes is the usual method in Lingnan area.The choice of TCM treatment is influenced by age,course of disease,clinical type,total score of secondary symptoms,tongue,tongue coating,pulse.It can be considered comprehensively in clinical application.
