| Objectives:Construct an incontinence-associated dermatitis nursing training program suitable for clinical nurses,and through the preliminary evaluation of its training effect.Specific goals:①Compile a questionnaire and investigate the level of knowledge and practice of nursing incontinence-associated dermatitis among clinical nurses in Yunnan Province;② Combine literature research,current situation investigation and Delphi method to construct a clinical nurse incontinence-associated dermatitis nursing training program for future incontinence-associated dermatitis training The implementation and development in clinical nurses provide theoretical basis;③Evaluate the application effect of clinical nurses’ incontinence-associated dermatitis nursing training program.Methods:This research consists of 3 parts.Part 1:Investigation and research on the status quo of nursing knowledge,attitude and behavior of incontinence-associated dermatitis among clinical nurses in Yunnan Province.A self-made questionnaire was used to survey clinical nurses in 5 tertiary hospitals and 12 secondary hospitals in Yunnan Province.Using SPSS24.0 for data analysis,using statistical description methods to analyze general data,using two independent sample t-tests,one-way analysis of variance,and multiple linear regression analysis to compare and analyze questionnaire scores and influencing factors.Part 2:The construction of the nursing training program for clinical nurses incontinence-associated dermatitis.On the basis of literature research,combined with the survey results of the first part,the initial content of clinical nurses’ incontinence-associated dermatitis nursing training program is drawn up.Using the Delphi method,15 experts in the field were selected to conduct two rounds of expert consultation to revise the indicators,and finally determine the clinical nurses’ incontinence-associated dermatitis nursing training program.Part 3:The preliminary application of clinical nurses’ incontinence-associated dermatitis nursing training program.It is convenient to select 76 newly recruited nurses in our hospital as the research objects,and use the E-learning teaching method combined with the training program content constructed in the second part for training.The clinical nurse incontinence-associated dermatitis nursing knowledge and behavior questionnaire,knowledge test and training evaluation questionnaire were used to evaluate the training effect.Results:Part 1:The questionnaire includes 3 dimensions and 43 items,the total Cronbach’s a coefficient of the questionnaire is 0.955,and the content validity index S-CVI is 0.961,indicating that the questionnaire has good reliability and validity.A total of 1208 clinical nurses were effectively investigated.The knowledge dimension score was(11.14±2.67),the attitude dimension score was(60.65±7.55),the behavior score was(58.87±9.50),and the total questionnaire score was(130.66±).16.67)points.The results of one-way analysis of variance show whether you are a member of the wound stoma incontinence specialist group,whether you have cared for I AD patients,whether you know IAD-related guidelines or consensus,the number of IAD-related trainings,whether there are clinical nurses in the department for IAD prevention and treatment procedures,IAD knowledge,attitude,behavior scores and total scores of knowledge,belief and behavior are also different,and the difference is statistically significant(P<0.05).Multivariate linear regression analysis showed that the factors affecting the total score of clinical nurses’ incontinence-associated dermatitis nursing knowledge were:title,position,understanding of IAD-related nursing procedures or guidelines,IAD-related content training times,and whether the department has IAD prevention and treatment procedures.Part 2:Based on literature review,theoretical research,and current status investigation,the "Clinical Nurse Incontinence-associated dermatitis Nursing Training Program" is constructed.The positive coefficients of the two rounds of expert consultation were 100%,and the expert authority coefficients were 0.89 and 0.91 respectively.After the second round of consultation,the Kendall harmony coefficients of indicators at all levels were 0.400,0.273,0.268,and 0.339,respectively.The indicators were screened and revised according to expert opinions,and finally a clinical nurse incontinence-associated dermatitis nursing training program was formed.Part 3:1.Clinical nurse IAD knowledge,belief and behavior score:After training,the knowledge,attitude,behavior dimension scores and total scores were significantly higher than those before training,and the difference was statistically significant(P<0.05);2.Knowledge test score:the highest the score is 125 points,the lowest score is 55 points,the average score is(94.74±19.32),and the number of passers is 67(88.16%).3.Training evaluation questionnaire score:training content(4.666±0.45),training setting(4.55±0.50),teaching method(4.65±0.45),teaching staff(4.59±0.47),overall evaluation(4.56±0.46),average of all items the score is higher than 4 points.Conclusions:Nursing managers should pay attention to the level of knowledge,belief and practice of clinical nurses in IAD nursing,and especially should pay attention to providing training for clinical nurses with low seniority and low professional titles.This study is based on literature review,current situation investigation and the Delphi method to construct a clinical nurse incontinence-related dermatitis training program,including 3 first-level indicators,13 second-level indicators,57 third-level indicators,and 13 items for curriculum implementation evaluation.It is scientific and reliable.Empirical studies have found that it has strong applicability and good feasibility.It can significantly improve the knowledge,attitude and behavior of newly recruited nurses,and it will have a positive effect on the promotion of IAD nursing practice in clinical work in the future. |
