ObjectiveA randomized and controlled was conducted using Professor Zhuang Lixing’s regulating spirit acupuncture technique to investigate its application value in the treatment of post-stroke cognitive impairment, and to observe its clinical effectiveness, safety and impact on cognitive function improvement to provide a theoretical basis for clinical practice.MethodsUsing a randomized controlled clinical study, patients with post-stroke cognitive impairment(PSCI)who met the nadir criteria were randomly divided into 2 groups, with 33 cases in each group. Both groups were treated with medication, rehabilitation, and conventional acupuncture for stroke disease. The treatment group was treated with regulating spirit acupuncture technique, and the main points included Si Shen acupuncture,Zhi San acupuncture(Shen Ting,Ben Shen),Yin Tang,He Gu,Shen Men,Shen Yi,Zhao Hai,Tai Chong, and San Yin Jiao. The control group was treated with normal acupuncture, and the main points included Baihui,Yin Tang,Shenting, and Si Shen Cong,Tai Xi,and Hang Zhong. Both groups were treated for 5 days per week with 2 days of rest for 4 weeks. Before and after treatment, the patients were scored on scales including the Mini-mental State Examination, the Montreal Cognitive Assessment and the Modified Barthel Index to compare the clinical efficacy and adverse effects of the regulating spirit acupuncture technique and the ordinary acupuncture method in the treatment of PSCI.The clinical observation data were analyzed by SPSS21.0.The test level was set to a=0.05(P≤0.05).It was considered that the difference was statistically significant.Results1.A total of 66 patients with PSCI who met the criteria in the inpatient or outpatient departments of the Department of Acupuncture and Moxibustion,Rehabilitation Center, and Department of Encephalopathy of the hospital from October 2019 to January 2021 were included in this study,of which a total of 3 cases were dislodged and the number of valid cases was 63,including 32 cases in the treatment group and 31 cases in the control group.2.Baseline comparison:The two groups were comparable before treatment,in terms of gender, age, duration of illness,Mini-mental State Examination,Montreal Cognitive Assessment and modified Barthel Index score(P>0.05).3.Clinical efficacy:Comparison was made between the two groups, the treatment group was significantly effective in 11 cases, effective in 18 cases, ineffective in 3 cases, and the total effective rate was 90.63%.The control group was evidently effective in 5 cases, effective in 18 cases, ineffective in 8 cases, and the total effective rate was 74.19%.There was significant difference between the two groups(P<0.05).4.Comparison of Mini-mental State Examination:The differences between the two groups were statistically significant(P<0.05)both in terms of within-group comparisons before and after treatment, and in terms of between-group comparisons and the difference before and after treatment, and after treatment differences. Both groups were able to improve the patients’ intelligence and cognition, and the efficacy of the regulating spirit acupuncture technique group was better than that of the general acupuncture group.5.Comparison of Montreal Cognitive Assessment:The differences were statistically significant(P<0.05)when comparing the two groups before and after treatment, and when comparing the scores of the difference before and after treatment, and when comparing the scores of the two groups after treatment. Both groups were able to improve cognitive function, and the efficacy was better in the regulating spirit acupuncture technique group.6.Comparison of modified Barthel index scores:The difference was statistically significant(P<0.05)when comparing before and after treatment within the group;the difference was not statistically significant(P>0.05)when comparing the two groups after treatment. Both groups could improve the patients’ ability of daily living.7.Safety assessment:During the trial, there were no significant adverse reactions in the two groups.ConclusionTo sum up,Both regulating spirit acupuncture technique and general acupuncture are effective in treating post-stroke cognitive impairment, and both can improve cognition and daily living ability with high safety. In terms of cognitive improvement and overall efficacy, the regulating spirit acupuncture technique was more effective. The present study provides a basis for the improvement of cognitive function in PSCI by Tiao Shen Acupuncture, which is worthy of further clinical promotion.
