ObjectiveTo observe the clinical efficacy of removing blood stasis and generating new blood needle technique combined with acupoint injection in the treatment of vascular vertigo after stroke.MethodsPatients with vertigo during recovery from cerebral infarction in the Department of Brain Diseases and Rehabilitation Department of Shenzhen Bao’an Traditional Chinese Medicine Hospital,were selected into the study.The included 72 patients who met the inclusion and exclusion criteria were randomly divided into removing blood stasis and generating new blood needle technique combined with compound angelica injection group(combined group)and betahistine hydrochloride group(drug group)and 36 cases in each group.Because of missing and dropping out 4 cases and 2 cases in each group,there were 34 cases in each group.On the basis of basic treatment,all cases were given acupuncture and acupoint injection of compound angelica injection in the combined group.For acupoint injection,pick bilateral Fengchi and inject 1ml of angelica injection on each side once a day for 7days.For acupuncture,pick bilateral Sanyinjiao,Yinlingquan,Zusanli,Xuehai and manipulate needles in reinforcing method.Manipulate needles on Quchi,Hegu,Taichong,Fenglong in reducing method.Fengchi,Tianzhu,and Baihui were given mild reinforcing and reducing needle technique.All needles were retained on points for 30 minutes after arrival of qi.Acupuncture was manipulated once every 10 minutes per day for 7 days.The drug group was given betahistine hydrochloride 1 tablet every 8 hours per day for 7 days.Before and after treatment,the patients were evaluated with Dizziness Clinical Symptom Rating Scale,Dizziness Assessment Rating Scale(DARS),Dizziness Handicap Inventory-Simple(DHI-S)and the mean volecity(Vm)and pulsatility index(PI)of Transcranial Doppler Ultrasound(TCD).Finally,the curative effect was evaluated according to the score of the Dizziness Clinical Symptom Rating Scale.Results1.After treatment,the scores and results of Dizziness Clinical Symptom Rating Scale,DHI-S,DARS,PI of TCD on left and right vertebral arteries and basilar artery of the two groups were significantly lower than those before treatment(P<0.01),and scores of Vm of TCD on three ateries were significantly higher than those before treatment(P<0.01).After the treatment,the score of combined group has a greater decline in the Dizziness Clinical Symptom Rating Scale,DARS and Vm,and there is a statistical difference compared with drug group(P<0.05).The two therapies has no difference on reducing the scores of DHI-S and improving the PI of TCD(P>0.05).2.After treatment,the total effective rate of the combined group was 91.2%compared with the 70.6%of the drug group.The combination group performed better than the drug group in terms of efficacy,and there was a significant difference(P<0.001)3.Based on the basic treatment,the revoming blood statsis and generating new blood needle technique combined with compound angelica injection is more effective in treating vascular vertigo after stroke.ConclusionFor patients with vascular vertigo after stroke,the treatment with removing blood stasis and generating new blood needle technique combined with compound angelica is safe,simple and effective,and the curative effect is significantly better than that of taking betahistine hydrochloride tablet.It improved symptoms and the mean blood flow velocity and pulsatility index of vertebral and basilar arteries.symptoms.The therapy has more advantages in improving the quality of life during the rehabilitation period,and have more significant effects.
