| ObjectiveThrough retrospective study,collecting clinical cases with chronic atrophic gastritis as the first diagnosis,collating the general information,clinical symptoms,TCM syndromes and TCM medication of the patients,statistically analyzing the distribution of TCM syndromes and the rules of TCM medication in the treatment of chronic atrophic gastritis.MethodsAccording to strict standards,1052 cases with a total of 1625 consultations of chronic atrophic gastritis cases from January 2019 to June 2020 were screened.Collecting the general information,symptoms,tongue and pulse appearances,TCM diagnosis,TCM syndromes and TCM prescriptions and the names of traditional Chinese medicine were standardized,then,all data was recorded into Excel software and sorted into a format that could be recognized by the software of TCM inheritance calculation platform.Finally,the frequency statistical analysis,association rule analysis and cluster analysis functions of the statistical platform were used to conduct statistical analysis on the collected data.Results1.General data evaluation:in 1052 cases,547 female patients and 505 male patients were included,with no significant difference in gender ratio.The patients were concentrated in the age group of 41-50 years old,51-60 years old and 61-70 years old,accounting for 82.5%of total patients.2.Symptom analysis:75 clinical symptoms were extracted by frequency analysis,clinical symptoms among the top few were epigastric distention,epigastric pain,belching.dry mouth,insomnia,fatigue,dizziness,acid reflux,poor appetite,indicating that the diagnosis of this disease consist of primary symptom of digestive system like epigastric distention,epigastric pain,secondary symptom of digestive system like belching,acid reflux,poor appetite and secondary symptom system like fatigue,dry mouth,insomnia,dizziness.3.The disease of TCM analysis:those cases involving 20 diseases of traditional Chinese medicine,the frequency of the top six were all the digestive system diseases,they respectively are stomach distention,stomachache,abdominal pain,abdominal distention,diarrhea disease,gastroesophageal reflux diease.One of the highest frequency was the stomach distention(0.563),the second was stomachache(0.332),indicating that chronic atrophic gastritis’s traditional Chinese medicine diagnosis were mainly stomach distention and stomachache.4.TCM syndromes Analysis:A total of 29 syndromes were counted out in this study,among which the top four in frequency were spleen deficiency and qi stagnation(33.0%),spleen deficiency and dampness accumulation(25.4%),deficiency and excessive syndrome(14.6%),and liver stagnation and spleen deficiency(11.4%),accounting for 84.4%in frequency.5.Rules of TCM medication:A total of 296 TCM flavors were counted in this study,with a total of 25,888 times of use,mainly including warm and smooth herbs and sweet,spicy and bitter herbs,which were mostly distributed in the spleen and stomach meridian,liver meridian,kidney meridian and lung meridian.Among them,the herbs for tonifying deficiency were used the most(7060 times),followed by the medicine for promoting blood circulation and regulating qi.The top five herbs in the list of use frequency were Atractylodes(1332 times),Licorice(1135),Zedoary(1074),Dangshen(1058)and Panax notoginseng(975).ConclusionClinical patients with chronic atrophic gastritis is given priority to with spleen deficient,and mixed with the excessive,close to the spleen,stomach and liver system.Therefore,when diagnosing and judging the TCM syndromes,both the digestive symptom and constitutional symptom were required to notice.On treatment,it should be based on the transportation of spleen and stomach,also pay attention to the liver wood,attaches great importance to the regulation of qi activity according to different pathogenesis like qi stagnation,dampness,blood stasis etc for syndrome differentiation and treatment. |
