Clinical Study On Buzhijianyi Prescription In Treating Vascular Dementia With Will And Ambitionl Deficiency

Objective: Based on Professor Teng Jing’s clinical experience for many years and the previous research of the research group,from the perspective of "Five Minds" theory of Chinese medicine,this topic found that the pathogenesis characteristics of vascular dementia with the same symptoms can be better explained from the perspective of lack of will and ambitionl,in which the symptoms of lack of will and ambitionl are more common,and the method of strong will and ambitionl and determination is often used in treatment,and the clinical efficacy and improvement of TCM syndromes of Buzhijianyifang in treating vascular dementia with insufficient will and ambitionl are observed,in order to provide "Five Minds" of Chinese medicine for treating vascular dementia.Methods: From January 2019 to September 2020,60 patients with vascular dementia who met the inclusion criteria were screened in the Affiliated Hospital of Shandong University of Traditional Chinese Medicine and randomly divided into experimental group and control group,with 30 cases in each group.The experimental group was treated with Buzhi Jianyi prescription(ginseng 9g licorice 6g poria cocos 15 g polygala tenuifolia 6g Acorus gramineus 6g Achyranthes 6g longan pulp 20 g Cistanche deserticola 12 g Atractylodes macrocephala 15 g Saposhnikovia 12 g Albizia julibrissin 6g)combined with donepezil hydrochloride tablets,while the control group was treated with donepezil hydrochloride tablets for 12 weeks.Before and after treatment,the self-made Chinese Medicine Five-God Illness Scale,Alzheimer’s Disease Cognitive Function Assessment Scale(ADAS-Cog),Mini Mental State Scale(MMSE)and Daily Living Ability Scale(ADL)were filled out to evaluate the improvement of patients’ cognitive ability,daily living ability and Chinese Medicine Five-God Syndrome.After treatment,the clinical data of patients were collected,and the data were analyzed by SPSS 26.0,and the conclusion was drawn.Results:1.General data: There is no significant difference in sex,age and course of disease between the two groups(P>0.05),which is comparable.2.TCM syndrome judgment(1)The effective rate of TCM syndrome in the control group was26.67%,while that in the experimental group was 63.33%,which was significantly higher than that in the control group.(2)Comparison of TCM "insufficient will and ambitionl" syndrome score and its components: After treatment,the TCM "insufficient will and ambitionl" syndrome scores of the two groups were lower than before(P <0.05),and the experimental group was obviously superior to the control group,with statistical difference(P < 0.05).After treatment,the control group had statistical differences in the scores of "forgetfulness,near-term memory loss","slow response and dullness".After treatment,the scores of TCM syndromes in the experimental group were as follows: "low voice","lack of perseverance,perseverance,easy to be discouraged","dizziness,blurred vision,dizzy","forgetfulness,near-term memory loss","trance,dull mind,unclear","slow response,dullness","constipation","low back pain","low back pain" The improvement degree of TCM syndromes in the experimental group,such as "lack of perseverance,perseverance,easy to be discouraged","dizziness,dim vision,dizzy","forgetfulness,memory loss in recent events","trance,drowsiness,unclear mind","difficulty in falling asleep,easy to wake up,poor sleep quality" and "cold hands and feet",was better than that in the control group(P It shows that donepezil hydrochloride tablets combined with Buzhi Jianyi prescription can improve TCM syndrome score more obviously than oral donepezil hydrochloride tablets alone,so donepezil hydrochloride tablets combined with Buzhi Jianyi prescription has better clinical efficacy in treating insufficient will and ambitionl type vascular dementia.3.Western medicine curative effect judgment(1)the effective rates of ADAS-Cog,MMSE and ADL in the control group were 40.00%,43.33% and 20.00% respectively,while those in the experimental group were 56.67%,70.00% and 36.67% respectively.The clinical effective rate of the experimental group was higher than that of the control group.(2)After treatment,the ADAS-Cog,MMSE scores and ADL scores of the two groups were significantly improved within and between groups(P <0.05),indicating that donepezil hydrochloride tablets combined with Buzhi Jianyi prescription and oral donepezil hydrochloride tablets alone can improve patients’ cognitive function and daily living ability,and the improvement degree of the experimental group is obviously better than that of the control group.4.Safety assessmentNo significant abnormality was found in routine examination and test indexes.Conclusion: Buzhijianyi Decoction combined with Donepezil Hydrochloride Tablets has a significant clinical effect in the treatment of vascular dementia with insufficient will and ambitionl.Compared with western medicine alone,it can further improve the cognitive function,daily living ability and abnormal symptoms of mental and psychological behavior of patients with vascular dementia,and indeed improve the curative effect of five minds syndrome in traditional Chinese medicine.It is safe to use in clinical medicine and has clinical promotion value.
