Objective:Through the clinical observation of the curative effect of the internal and external combination of the method of strengthening and detoxification in the treatment of post-chemotherapy oral mucositis caused by 5-fluorouracil as the main regimen,to evaluate the relief and control of the treatment of post-chemotherapy oral mucositis,to provide more theoretical and clinical research basis for the treatment of post-chemotherapy oral mucositis caused by 5-fluorouracil by traditional Chinese medicine.Methods:All patients in this study were hospitalized in the Oncology Department of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from May 2020 to February 2021.Sixty patients who were treated with5-fluorouracil as the main chemotherapy regimen were selected strictly according to the inclusion criteria,and the patients were divided into observation group and control group with 30 cases in each by envelope method.The intervention began on the first day after the appearance of swelling and pain of oral mucosa after chemotherapy.The experimental group was treated with a combination of internal and external strengthening and detoxification method(oral administration of traditional Chinese medicine combined with external gargle of traditional Chinese medicine),while the control group was treated with a combination of gargle with metronidazole and gentamicin and oral multivitamin tablets.To observe the effect of combining internal and external treatment with strengthening and detoxifying method and comparing the effects of metronidazole plus genamicin mouthwash combined with oral compound vitamin tablet in improving the severity,pain,increasing white blood cells,shortening the duration of disease,and improving the clinical symptoms and quality of life of patients with chemotherapy-related oral mucositis,and analyze the data through SPSS 25.0.Results:(1)In the 60 patients included in the trial,there was no case of shedding.(2)There were no statistical differences in disease site distribution,gender,age,Karst score,surgical situation,liver and kidney toxicity between the two groups(P > 0.05).(3)Oral mucositis in the experimental group was graded as grade 0 in 22 cases,grade 1 in 4 cases,grade 2 in 3cases,grade 3 in 1 case,grade 4 in 0 cases,and the total effective rate was 86.7%.The grade of oral mucositis in the control group was grade 0 in11 cases,grade 1 in 11 cases,grade 2 in 7 cases,grade 3 in 1 case,grade4 in 0 cases,and the total effective rate was 66.7%.The results showed that the experimental group was better than the control group in improving the severity of mucositis,and the difference was statistically significant(P< 0.05).(4)In terms of pain improvement,the proportions of painless,mild,moderate and severe pain in the experimental group were 73.33%,26.67% and0%,while those in the control group were 53.33%,40.00% and 6.67%,respectively,indicating that Fuzheng and Jiedu method had the effect of alleviating pain in patients(P < 0.05).(5)In terms of the effect on white blood cells,the white blood cell count in the experimental group was(3.64±1.38)×109/L,and that in the control group was(2.95±1.01)×109/L.The results showed that the increase of white blood cells in the experimental group was better than that in the control group,and the difference was statistically significant(P < 0.05).(6)The average ulceration period was(6.13 ± 1.85)days in the control group and(3.97 ± 1.13)days in the experimental group,indicating that the Fuzheng and Jiedu method can significantly shorten the duration of the disease and promote wound repair(P < 0.05).(7)In terms of improving TCM symptom score,the effective rate of the observation group was 73.33%,which was statistically different from40.00% of the control group(P < 0.05),suggesting that Fuzheng Jiedu method can reduce patients’ systemic symptoms.Conclusion:The curative effect of 5-fluorouracil on oral mucositis after chemotherapy was significant.Compared with the western medicine group,the pain and severity caused by oral mucositis were significantly reduced and the healing time of ulcer was shortened.At the same time,it can also improve the systemic symptoms of patients after chemotherapy and increase the white blood cells(P < 0.05),so that patients can benefit from treatment,with high safety and high clinical application value.
