Clinical Study On The Treatment Of KOA By Massage Of Zhuang Medicine Based On "Jingu Balance"

Objective:Objective to elucidate the effective mechanism of muscle massage of Zhuang medicine in restoring "muscle bone balance" of knee joint,and to explore the best scheme for the treatment of knee osteoarthritis.Methods: 125 cases of knee osteoarthritis patients meeting the inclusion criteria were selected and divided into 25 cases in the treatment tendons group,25 cases in the treatment bone group,25 cases in the first tendon and back bone group,25 cases in the first bone and back tendon group,and 25 cases in the tendons and bone treatment group.The tendon treatment group was intervened by massage of Zhuang medicine;The bone treatment group was intervened by stretch therapy;The first tendon and back bone group first course of treatment was intervened by massage of Zhuang medicine,the second course was intervened by stretch therapy;The first bone and back tendon group first course of treatment was intervened by stretch therapy,the second course was intervened by massage of Zhuang medicine;The same treatment group was intervened by massage of Zhuang medicine combined with stretch therapy In advance.The treatment is performed once a day,once every 2 days,3times/week,6 times/course,for a total of 2 courses.(1)VAS score was used to evaluate the degree of pain;(2)WOMAC osteoarthritis index was used to evaluate the knee function.(3)The clinical efficacy was determined by the standard of TCM disease and syndrome diagnosis.The clinical treatment results and data of clinical subjects were statistically analyzed and compared by spss22.0.Results: A total of 125 KOA patients were included in the study,14 cases fell out,and 111 cases were finally completed.(1)Baseline data comparison: there was no significant difference in age,gender and course of disease before treatment(P > 0.05).(2)Pain score comparison: VAS score of patients before treatment had no significant difference(P > 0.05),which was comparable.After treatment,VAS scores were significantly lower than before treatment,the difference was statistically significant(P < 0.05).Pairwise comparison showed that there was no significant difference between the first tendon and then the bone group and the tendon and bone treatment group(P >0.05),But compared with the other three groups,the VAS score decreased significantly(P<0.05).(3)Knee function score: WOMAC Osteoarthritis Index score of patients before treatment had no significant difference(P > 0.05),which was comparable.After treatment,WOMAC Osteoarthritis Index scores were significantly lower than before treatment(P < 0.05).Pairwise comparison analysis showed that the functional score of the tendon before bone group was significantly lower than that of the other four groups(P <0.05).(4)Overall curative effect comparison: After treatment,the total effective rate of the first tendon and then bone group is 95.24%,the total treatment rate in the treatment of muscles and bones was 91.30%,the total effective rate of the bones and tendons group is 85.71%,the total effective rate of the tendon treatment group is 83.33%,the total effective rate of the bone treatment group is 63.63%,the differences were statistically significant(P <0.05).The comparison between the two groups showed that there was no significant difference between the first tendon and the bone group and the same treatment group(P > 0.05),but compared with the other three groups,there was statistical difference(P <0.05).Conclusions:(1)The breaking of the " Jingu balance " system of the knee is the order of muscles and bones,so the treatment sequence of "treating the muscles and bones later" should be followed.(2)Adopting the treatment plan of “treating the tendons first,regulating the bones later”can restore the state of “balance of muscles and bones” to the greatest extent and improve the clinical symptoms of patients.(3)The massage of Zhuang medicine can adjust the "Jingu imbalance" of the knee by intervening in "Shujin",so as to achieve the therapeutic effect of KOA.
