Study On The Intervention Effect Of Huayu Qushi Recipe On The Clinical Effect Of High Fibula Osteotomy In Patients With Knee Osteoarthritis

Objective: To observe the intervention effect of Huayu Qushi recipe on the clinical efficacy of high fibular osteotomy in patients with knee osteoarthritis(KOA),to verify the effect of this formula on the improvement of clinical symptoms and function of knee medial compartment osteoarthritis,and to explore the mechanism of Huayu Qushi recipe in the treatment of medial knee compartment osteoarthritis with phlegm stasis.Provide the best and reasonable scheme for the treatment of the disease,improve the pain symptoms of patients and reduce the adverse effects of the disease,and bring benefits to the rehabilitation treatment of the majority of middle-aged and elderly patients.Methods: A total of 64 patients with Medial Compartment Osteoarthritis who underwent surgery in the Department of Joint Trauma Orthopaedics,Xianghuyuan District,Guangxi Zhuang Autonomous region Hospital of traditional Chinese Medicine from January 2019 to December 2020 were randomly divided into two groups: group A(Huayu Qushi recipe combined with high fibula osteotomy)and group B(simple fibula high osteotomy group).High fibula osteotomy was performed by the same group of visiting staff or above.We need to collect and analyze the data of operation time,intraoperative blood loss and postoperative hospital stay.To evaluate the blood stasis state of patients,five items of blood coagulation were detected before and after operation.X-ray films of positive and lateral position of knee joint and fulllength X-ray films of both lower limbs were performed before and after operation to check the structural changes of knee joint of affected limb.The tibiofemoral angle,the medial and medial knee joint space were measured before and 3 months after operation.Also,we had to evaluate the VAS score,HSS score and knee joint function at the time of before operation and 2weeks,1 month and 3 months after operation.The results were analyzed by SPSS 29.0 software.Results: 1.Overall situation: there was no significant difference in age,sex,course of disease,unilateral and bilateral knee joint X-ray Kmurl grade,VAS score,HSS score,D-dimer,fibrinogen,thigh-tibial angle and joint space between group An and group B before operation,but there was no significant difference between the two groups.Of the 64 patients included in this study,60 patients cooperated with this study,4 cases fell off,and there were no serious adverse reactions.2.Intra-group comparison: the VAS score,HSS score,thigh-tibial angle and joint space of the two groups were significantly improved before and after treatment,which indicated that Huayu Qushi recipe combined with high fibula osteotomy and simple high fibula osteotomy could effectively relieve joint pain,improve joint function and correct the strength line of lower extremities.D-dimer and fibrinogen in both groups increased on the first day after operation,and the levels of D-dimer and fibrinogen in serum at 2 weeks after operation were lower than those on the first day after operation.However,after taking Huayu Qushi recipe,D-dimer and fibrinogen in group A decreased on the 3rd day,1 week and 2 weeks after operation,while Ddimer and fibrinogen in group B continued to increase on the 3rd day after operation,and decreased from 1 week and 2 weeks after operation,indicating that Huayu Qushi recipe can immediately improve the state of blood stasis after operation and achieve a possibility of rapid recovery,while the simple operation group can only rely on self-regulation to recover.3.Comparison between groups: at the end of treatment,the effective rate of the two groups was 100%.There was no significant difference in TKA angle and knee gap between the two groups of KOA patients before and after treatment.This shows that the main corrective effect on the lower limb force line of KOA is that HFO,Huayu Qushi prescription has relatively little or no effect on the force line correction of KOA.In the comparison of 2 weeks,1month and 3 months after operation,the improvement of VAS score,HSS score,D-dimer and fibrinogen in group B was lower than that in group A.This shows that high fibula osteotomy alone before and after treatment can also reduce pain and improve knee joint function in patients with KOA,but compared with Huayu Qushi recipe combined with HFO treatment,the therapeutic effect has a certain disadvantage.Conclusion(s): 1.Huayu Qushi recipe combined with high fibula osteotomy and simple high fibula osteotomy are reliable in the treatment of medial knee compartment osteoarthritis,which can reduce pain and improve knee joint function and other clinical symptoms.Huayu Qushi recipe combined with high fibula osteotomy is more effective in improving patients’ clinical symptoms.2.Huayu Qushi recipe can not only improve the metabolic microenvironment of patients with knee osteoarthritis,this study also proved that Huayu Qushi recipe can improve the blood coagulation state of patients with knee osteoarthritis.3.The main corrective effect on the lower limb force line of knee osteoarthritis is the high fibula osteotomy,while the traditional Chinese medicine treatment of removing blood stasis and removing dampness prescription has no obvious effect on the force line correction of knee osteoarthritis in the short term.4.No serious adverse reactions were found in group An and group B,which proved the overall safety of this study.
