Objective:Since my practice is based in Canada,many patients do not accept acupuncture and prescription due to their belief and cultural background.Therefore,I hope to analyze the external treatment of lumbago through the analysis of traditional Chinese medicine books,sorting out and analyzing the literature about lumbago in classic Chinese medicine books,and getting the purest external treatment method from the classic Chinese medicine books.Ultimately,resulting in a faster and better method to solve the pain for patients.Methods:The Chinese Medical Code contains 1156 ancient medical books of the past dynasties and collected the main works of traditional Chinese medicine before liberation.We summarized all the documents about lumbago in the classic Chinese medicine books and established a complete database.The database was sorted out into various categories:The external treatment methods and analyzed the frequency of the external treatment,studied the external treatment of lumbago,the classification,etiology and pathogenesis,treatment principles,treatment,external treatment,medicine,acupoints,special tools,and the evolution and development of external treatment.Result:1.From the summary of the documents recorded,from the Western Han Dynasty to the Qing Dynasty,the research on lumbago was on the rise.Especially during the Ming and Qing Dynasties,a large number of treatises on lumbago were written.The research on lumbago disease expanded deeper,more detailed dialectical classification,and more types of external treatment and flexibility.This spirit of innovation and development was very worth our study.2.The literature data show that the word "Yaotong" was first found in the Yin Yang and Ten Pulse Moxibustion Sutra unearthed in Mawangdui in the Western Han Dynasty.The Inner Classics also has the names of "back pain","lumbago and leg pain",and the synopsis of the golden chamber also has the related treatise of kidney.In the Sui Dynasty,the "origin and waiting for diseases" of Chaoyuan Fang in Sui Dynasty began a special discussion on the lumbago diseases.Then,the author also discussed the "back and back diseases" as the special treatise of lumbago.The Tang,Song,Ming and Qing Dynasties then expanded and created more research on lumbago than the previous dynasties.3.The Internal Classics points out that the general pathogenesis of Bi syndrome was "pain without failure" and "pain without honor".This was mainly due to the external sensation,six masturbation and insufficient internal injury.Wang KENTANG from Ming Dynasty divides the lumbago into emotional failure(internal cause),six classics lumbago(external cause)and long-term kidney injury(non internal and external causes);Zhang Jingyue in Ming Dynasty divides the cause of back pain into insufficient of Yang,overworked on the kidney,and external injuries.Doctors later found themselves unable to take away from the above-mentioned patterns and the etiology of lumbago was similar in various ways.4.There were 57 documents about external treatment methods included in the study.There were 7 external treatment methods recorded,which includes moxibustion,external application,Qigong guidance,massage,acupoint administration,scraping and fumigation.The past generations also choose appropriate external treatment methods according to the syndromes.Amongst the methods,moxibustion is the most commonly used by doctors,accounting for 50.88%of all external methods.Simultaneously with moxibustion,the Qigong guidance therapy was used as well.The syndromes of qi stagnation and blood stasis can only be treated with Qigong guidance,while moxibustion has a stronger applicability.It also shows that the different external treatment methods will provide various results depending on the syndromes.Conclusion:Through the analysis and rearrangement of classic literatures on lumbago syndrome and comparing with the modern medicine treatment method,the following conclusions are drawn:1.The most common type of lumbago was kidney yang deficiency and low back pain,followed by qi stagnation,blood stasis and back pain,which accounted for 43.86%of all the cases;2.Moxibustion is the most used in all external treatment methods,accounting for 50.88%;3.Based on records,it is clear that the study of lumbago was built on a solid foundation by doctors of past dynasties.As time moved by,doctors used the foundation and further innovated the understanding of the study on lumbago.This allowed the understanding of the lumago to deepen and become more abundant.4.There are many restrictions on practicing medicine in Canada,many patients have insufficient knowledge of acupuncture and traditional Chinese medicine.Hence I need to find various ways to treat patients to relieve the patient’s pain.In the process of learning and practice,I found that applying heat after massage and acupuncture with electrical therapy have the best effect.The disadvantage is that the treatment time is longer,generally it takes 60 minutes or more.Outlook and recommendations:1.Based on the belief and cultural background,compared with acupuncture and traditional Chinese medicine,other therapy of traditional Chinese medicine is easy to be accepted abroad.Therefore,we should continue to dig out the classic Chinese medicine books,find more treatments,and make a contribution to the inheritance and innovation of traditional Chinese medicine.2.In view of the fact that some patients can not accept acupuncture and prescription,it is suggested that more therapy can be added in the modern Chinese medicine textbooks for foreign students.This will allow foreign students to learn and to be used in clinical practice in the future,resulting in better results based on specific conditions.
