Objective:In order to explore the metabonomics characteristics of neonatal jaundice with syndrome of interior dampness-heat,the change pattern were analyzed by LC-MS metabonomics.Through the study,it reveals the effect of Traditional Chinese medicine enema prescription on the characteristic markers of the neonatal jaundices with syndrome of interior dampness-heat.And theoretical foundation for Qing Re Tui Huang enema prescription that treats neonatal jaundice can be provided.Methods:In this study,20 children with NJ meeting the diagnostic criteria and 10 healthy newborns were included.The neonatal jaundice children were randomly divided into two groups,control group(the western medicine conventional treatment group)and experimental group(combined Chinese medicine enema treatment group),10 cases in each group.Before and after treatment for 5 days,blood was collected in both group.Based on LC-MS metabonomics techniques and methods,multivariate statistical analysis(PCA analysis,OPLS-DA analysis)and univariate statistical analysis(Student’s t test)were used,combined with HMDB,KEGG and other databases,significant difference metabolites with important biological significance were screened to explore the metabolomic characteristics of dampness-heat syndrome and the mechanism of Qing Re Tui Huang enema prescription.Results:1.Comparison of the metabolomic results of neonatal jaundice with Syndrome of interior dampness-heat and healthy neonates showed that the distribution of metabolites in plasma samples of the two groups was completely different through multivariate statistical analysis.Through the analysis and screening of metabolites,a total of 21 kinds of differential metabolites ware obtained.The contents of α-ketoglutaric acid,cis aconic acid,2-oxoglutaric acid and citric acid were down-regulated,while the contents of uric acid,creatinine,pantothenic acid,niacin and uridine were up-regulated.2.According to the results of metabonomics in the experimental group,multivariate statistical analysis was conducted to construct PCA and OPLS-DA models for the experimental group before and after treatment,and corresponding metabolic profiles were obtained.It was found that plasma metabolites of the trial group before and after treatment could be distinguished.Through the analysis and screening of metabolites,a total of 17 kinds of differential metabolites were obtained.The contents of α-ketoglutaric acid,2-aminoadipic acid,cis-aconitonic acid,N-acetylmannolamine,choline and acetylcholine were up-regulated,while the contents of uric acid,creatinine and pantothenic acid were down-regulated.3.After treatment,the comparison of the experimental group and the control group showed that multivariate statistical analysis was conducted to construct PC A and OPLS-DA models for the control group and the experimental group,and corresponding metabolic profiles were obtained.It was found that the control group and the experimental group were aggregated and separated.Through the analysis and screening of metabolites,a total of 15 different metabolites were obtained.Alpha-Ketoglutaric acid,N-Acetyl-L-aspartic acid,L-Glutamic acid,choline and acetylcholine increased in the experimental group,furthermore,uric acid and creatine decreased in the experimental group.Conclusion:1.The LC-MS technique can be used to analyze the metabolites in the plasma of neonatal jaundice.In addition,the metabolic characteristics of neonatal jaundice with syndrome of interior dampness-heat can be distinguished and explained from those of normal neonates,as well as before and after treatment.2.There are mainly amino acid and energy metabolism disorders in the plasma of neonatal jaundice with dampness-heat syndrome.The screened differential metabolites can be used as potential biomarkers for the diagnosis of neonatal jaundice with dampness-heat syndrome.3.The mechanism of Qing Re Tui Huang enema prescription on neonatal jaundice might be connected with three aspects,including the regulation of amino acid,energy metabolism and cholinergic pathway.
