[Objective]Investigate the current situation of orthopedic nursing human resources,dynamically measure the direct and indirect nursing working hours of orthopedic patients during different hospitalization days,calculate the average nursing working hours of patients at different levels of care,and clarify the nursing service items and minimum executive qualifications under different levels of care,and construct The model of human resource allocation for orthopedic nursing in third-grade class A hospitals of traditional Chinese medicine provides theoretical support for the allocation of clinical nursing human resources and the hierarchical management of nurses.[Method](1)Through literature review,to understand the current situation and existing problems of nursing human resource allocation in China,to analyze the causes of the problems,and to build a theoretical model of nursing human resources allocation in orthopedics department.(2)The questionnaire survey method was used to investigate the current status of nursing human resources allocation in orthopedic wards of two third-grade class A hospitals of traditional Chinese medicine in Jiangsu Province and Yunnan Province;the use of nursing information collection and management WeChat mini-programs to dynamically measure the nursing items and nursing items of different hospital days during the hospital stay Direct nursing hours and indirect nursing hours in orthopedic wards are calculated to calculate the average nursing hours for patients with different levels of care.(3)Based on the actual nursing service items measured in the field survey,the expert correspondence method is used to determine the nursing service items of patients with different levels of care and the minimum qualifications of the nursing staff,and combined with the results of nursing man-hour measurement,calculate the different qualifications required by patients of different levels of care The ratio of human-employee hours can be used to obtain the ratio of nurses with different qualifications required by patients with different levels of care.(4)Based on the average nursing working hours of patients with different nursing levels in orthopedics and the ratio of different qualified nursing staff required by patients with different nursing levels,the human resources allocation formula is used to establish a hierarchical configuration model of orthopedic nursing manpower in third-grade class A hospitals of traditional Chinese medicine.[Result](1)The current situation of orthopedic nursing human resources allocation in third-grade class A hospitals of traditional Chinese medicine:the overall nursing team is characterized by younger age,low professional title,low work experience and lack of highly educated nursing personnel.Nurses aged 35 and under accounted for more than half,68 per cent.The number of nurses and nurses accounted for 68%of the total number of nurses.40%of nurses have worked for less than five years.96%of the nurses have bachelor’s degree,and the nursing talents with master’s degree and doctor’s degree are still vacant.(2)Measurement results of nursing hours for patients with different nursing grades in the orthopedic department:This study investigated the nursing hours of 78 patients in the orthopedic department of third-grade class A hospitals of traditional Chinese medicine,with a total of 756 days in hospital.Among them,the average daily direct nursing hours of patients with special nursing,primary nursing,secondary nursing and tertiary nursing were 118.87mins,76.69mins,44.77mins and 28.82mins respectively,and the difference among different grades was statistically significant(P<0.001).Indirect nursing hours measurement results:Document processing accounted for nearly half of the total indirect nursing hours,which was 43.09%.The average daily indirect nursing hours of orthopedic patients are about 42.72mins.(3)Research results of nursing project and minimum executive qualification of patients:This study adopts the method of Delphi expert enquiry for 23 experts 2 rounds of enquiry,two rounds of enquiry for expert positive coefficients was 100%,the authority coefficient were 0.879,0.884,finally,102 special nursing items,102 first-level nursing items,91 second-level nursing items,and 76 third-level nursing items were determined,as well as the minimum nursing staff implementation qualifications for these nursing service items.(4)The model of orthopedic nursing human allocation in a third-grade class A hospitals of traditional Chinese medicine:The total number of nursing personnel that should be allocated daily is:(A×118.87+B×76.69+C×44.77+D×28.82+N×42.72)/480×1.23×1.5The number of daily supervisor nurses:Total number of daily nursing staffs×(34.12%A+10.13%B+2.79%C)/NThe number of daily senior nurses:Total number of daily nursing staffs×(20.85%A+26.52%B+9.34%C+9.86%D)/NThe number of daily nurses:Total number of daily nursing staffs×(44.01%A+59.86%B+78.89%C+80.29%D)/NThe number of daily assistant nurses:Total number of daily nursing staffs×(0.65%A+3%B+8.07%C+9.42%D)/NThe number of daily nursing workers:Total number of daily nursing staffs×(0.37%A+0.49%B+0.91%C+0.43%D)/NA,B,C,D and N are the number of special nursing,primary nursing,secondary nursing,tertiary nursing and the total number of patients in turn.[Conclusion](1)The direct nursing hours of patients with different nursing grades and hospitalization days in orthopedics department are different,which indicates that factors such as different conditions and hospitalization days become important contents of nursing human resources allocation.(2)This study determined the nursing service items under different nursing levels of patients and the minimum nursing staff execution qualifications through expert correspondence.Nursing services consisted of 102 items in 5 dimensions,covering comprehensive,specific,and specialized features,and were able to conduct manpower analysis for clinical nursing managers.Layer management,nurses provide guidance and reference for implementing precision nursing.(3)Based on the calculation of nursing workload and the evaluation of the nursing grade of orthopedic patients,combined with the expert correspondence method,the preliminary establishment of the a third-grade class A hospitals of traditional Chinese medicine orthopedic nursing manpower allocation model can provide a theoretical basis for the dynamic hierarchical nursing manpower allocation of nursing managers.
