Objective: In order to improve the quality of life for high-risk infants and their families,promote the safe and effective transition of high-risk infants,and provide evidences for enhancing care capacity of parents of high-risk infants,improving of hospital discharge preparation process of high-risk infants and making targeted plans of continuity of care by comparing readiness for hospital discharge and needs of continuity of care of parents of the high-risk infants in Yanbian region and analyzing influencing factors and the correlation between readiness for hospital discharge and needs of continuity of care.Methods: In this study,parents of high-risk infants admitted to a Grade III Level A hospital and a specialized hospital were selected for a questionnaire survey by convenience sampling from January 2020 to November 2020 in Yanbian,Jilin Province.The survey tools included the General Information Questionnaire,the Newborn Care Ability Questionnaire and the Needs of Continuity of Care of Parents of Preterm Infants Questionnaire.SPSS26.0 was used for data analysis,including descriptive analysis,independent sample t test,monofactor analysis,Pearson correlation and linear regression.Result:(1)In this study,the total score of readiness for hospital discharge of fathers of high-risk infants was 98.88±28.59,and average score was 2.30±0.66.The total score of readiness for hospital discharge of mothers of high-risk infants was 118.94±23.66,and the average score was 2.78±0.65.All scores show approximate normal distributions.(2)There were statistically significant differences in readiness for hospital discharge of fathers of high-risk infants among gestational ages,length of hospitalization,the order of births,caregivers after discharge,fathers’ ages,fathers’ education levels,family domiciles and monthly average income(P<0.05).There were statistically significant differences in readiness for hospital discharge of mothers of high-risk infants among gestational ages,birth weights,length of hospitalization,the order of births,caregivers after discharge,mothers’ ethnicities,mother’s ages,mothers’ education levels,family domiciles and monthly average income(P<0.05).(3)The total score of the father’s needs of continuity of care was 123.22±25.32,and the average score was 3.42±0.70.The total score of the mother’s needs of continuity care was 129.20±27.19,and the average score was 3.59±0.76.All scores show approximate normal distributions.(4)There were statistically significant differences in needs of continuity of care among gestational ages of high-risk infants,hospitalization days of high-risk infants,respondents of research,parents’ ages,parents’ education levels,parents’ occupations,the order of births of high-risk infants,and average monthly family income(P< 0.05).(5)There is a negative correlation between the needs of continuity care of parents of high-risk infants and the degree of readiness for hospital discharge,the dimension of care knowledge and the dimension of care skills.(6)The factors influencing needs of continuity of care included readiness for hospital discharge,educational levels and ages of parents.Conclusion:(1)The readiness for hospital discharge of fathers of the high-risk infants is medium.The readiness for hospital discharge of mothers of the high-risk infants is above the medium,which is higher.(2)Overall,the needs of continuity of care of parents of high-risk infants are high.The needs of continuity of care of mothers are higher than those of fathers.(3)The readiness for hospital discharge and ages of parents of high-risk infants have a significant negative impact on needs of continuity of care,while education level has a significant positive impact on needs of continuity of care.Parental readiness for hospital discharge is the strongest predictor of needs of continuity of care,followed by education level and age.(4)Hospitals should personalize discharge plans to improve readiness for hospital discharge of parents of high-risk infants,meet the needs of continuity of care,improve the quality of life of high-risk infants and their parents,ensure the physical growth and development of high-risk infants,reduce the readmission rate,and save medical resources.
