Objective: To explore the therapeutic value and safety of bronchoscope-based argon plasma coagulation(APC)combined with cryotherapy for airway tumors.Methods: From January 2016 to December 2020,the clinical data of patients with airway tumors who underwent bronchoscopic argon plasma coagulation(APC)combined with cryotherapy in the Department of Respiratory Medicine,Yijishan Hospital,Wannan Medical College,including the patient’s age,Gender,tumor pathological type,general condition,clinical symptoms,lung function,bronchoscopy performance and chest CT imaging,etc.,analyze and compare the clinical symptoms of patients before and after treatment,shortness of breath,health status score(KPS score),lung function,The degree of tracheal blockage,and the adverse effects of the patient’s treatment,to evaluate the effect and safety of APC combined with cryotherapy in patients with airway tumors.Results: 1.A total of 63 patients with airway tumors were included in this study,including 52 males and 12 females,with a male-to-female ratio of 4.33:1.The age range is between 20 and 76 years old,with an average age of 58.27 years old.2.Among 63 patients,49 cases(77.78%)of airway malignant tumors,46 cases of males and 3 cases of females,the main pathological type was lung squamous cell carcinoma 32 cases(50.79%).There were 14 cases(22.22%)of benign airway tumors,including 9 females and 5 males.The benign and malignant qualities of airway tumors are statistically different between men and women(P<0.001),and the differences between male and female genders are statistically significant for the pathological differences of different airway malignant tumors(P<0.05),and for different airway benign tumors There was no statistically significant difference in pathological types(P>0.05).3.The patient’s health status score(KPS score),shortness of breath level,and lung function(FVC,FEV1)before and after treatment were significantly improved,and the difference was statistically significant(P<0.001).4.The effective rate of airway recanalization(treatment)of APC combined with cryotherapy is 69.84%(85.70% for benign tumors and 65.30% for malignant tumors),and the effective remission degree of airway solid tumors is 61.90%(78.57% for benign tumors),Malignant tumor is 57.14%),the treatment efficiency of benign airway tumor and the effective remission of solid tumor are higher than that of airway malignant tumor.5.In this study,63 patients could tolerate bronchoscope argon plasma coagulation(APC)combined with cryotherapy.The main adverse reactions during the operation were bleeding,increased blood pressure,severe coughing,chest tightness,wheezing,and tachycardia.No serious complications or aggravation of the patient’s condition occurred.Conclusions: 1.Malignant tumors are more common in airway tumors,most of which are elderly male patients.Central lung squamous cell carcinoma is more common,and benign airway tumors are relatively rare.2.For benign airway tumors,APC combined with cryotherapy under bronchoscope can basically completely remove the tumor and achieve complete remission of the disease.For airway malignant tumors,APC combined with cryotherapy can achieve the goal of palliative tumor reduction,relieve symptoms of patients,and delay disease progression.3.APC combined with cryotherapy under bronchoscopy is safer,less traumatic,repetitive,and easy to operate.It can quickly and effectively relieve respiratory obstructive symptoms and improve the quality of life of patients.It is worthy of further promotion and application in clinical practice.
