【Objective】To investigate the status of health-promoting lifestyle and self-acceptance,family adaptability and cohesion of colorectal cancer survivors, analyze the correlation among the three variables, discuss the influencing factors of health-promoting lifestyle of colorectal cancer survivors, and test the mediating effect of self-acceptance between family adaptability and cohesion and health-promoting lifestyle. In order to provide theoretical and practical basis for medical staff to carry out the intervention of health-promoting lifestyle of colorectal cancer survivors and improve their quality of life.【Methods】In this study, the method of cross-sectional investigation was applied. From July 2020 to November 2020,a total of 216 colorectal cancer survivors in the ward and outpatient of Anorectal Department and Oncology Department in three Grade-A hospitals in Shandong Province were selected by convenience sampling method. The general information questionnaire, health promotion lifestyle profileⅡ,self-acceptance questionnaire and family adaptability and cohesion evaluation scales were used to conduct the questionnaire survey.Excel was used to input data, and SPSS21.0 and Amos23.0 were used for statistical analysis.Statistical methods include descriptive statistical analysis,Pearson correlation analysis, independent sample T-test,one-way ANOVA analysis, multiple linear stepwise regression analysis and constructing structural equation model.【Results】1.The total average score of health-promoting lifestyle of colorectal cancer survivors was(118.02±21.29),which was in the general level. The average scores of the six dimensions from high to low were spiritual growth(2.48 ± 0.65),nutrition(2.45 ± 0.50),stress management(2.42 ± 0.51),interpersonal relations(2.25±0.50),health responsibility(2.24±0.52)and physical activity(1.74±0.62);The total average score of self-acceptance was(39.84±5.42),including self-acceptance dimension(19.53±3.13)and self-evaluation dimension(20.31±3.32);The total average score of family adaptability and cohesion was(116.03 ± 15.93),of which the cohesion dimension was(68±8.61)and the adaptability dimension was(48.03±7.95).2.Pearson correlation analysis results showed that the total scores of health-promoting lifestyle and the scores of all dimensions were significantly positively correlated with the total scores of self-acceptance and the scores of all dimensions(P < 0.05).The total scores of health-promoting lifestyle and the scores of all dimensions were significantly positively correlated with the total scores of family adaptability and cohesion and the scores of all dimensions(P<0.01).The total scores of self-acceptance and the scores of all dimensions were significantly positively correlated with the total scores of family adaptability and cohesion and the scores of all dimensions(P<0.01).3.The results of independent sample T-test and one-way ANOVA analysis showed that there were significant differences in the scores of health-promoting lifestyle among colorectal cancer survivors with gender, education level, occupation, monthly family income, medical expenses payment and pathological stage, colostomy or not and postoperative time(P<0.05).4.Multiple linear stepwise regression analysis showed that education level, pathological stage, colostomy or not, postoperative duration,self-acceptance dimension and family adaptability dimension were the predictors of health-promoting lifestyle(P<0.05),which explained 65.7% of the variance.5.The structural equation model showed that self-acceptance played a partial mediating role between family adaptability and cohesion and health-promoting lifestyle. The standardized mediating effect value was 0.17,the direct effect value was 0.637,and the proportion of mediating effect was21.09%.【Conclusion】1.The health-promoting lifestyle of colorectal cancer survivors is at the average level,In the average score of each dimension, the spiritual growth dimension has the highest score, and the physical activity dimension has the lowest score. The total score of self-acceptance and the scores of each dimension were lower than the domestic norm. The score of family cohesion was higher than that of domestic norm, and the score of adaptability was lower than that of domestic norm.2.Health-promoting lifestyle, family adaptability and cohesion and self-acceptance are related to each other.3.The main factors influencing the health-promoting lifestyle of colorectal cancer survivors included education level, pathological stage, colostomy or not, postoperative time,self-acceptance dimension and family adaptability dimension.4.Self-acceptance plays a part of mediating role between family adaptability and cohesion and health-promoting lifestyle.Family adaptability and cohesion can not only directly affect the health-promoting lifestyle of colorectal cancer survivors, but also indirectly affect their health-promoting lifestyle through self-acceptance. |
