Objective:To explore the risk factors for bone metastasis in prostate cancer.Methods:Retrospective analysis of 148 cases of prostate cancer diagnosed by urology in the first Hospital of Jilin University from January 2018 to November 2020,All patients underwent prostate puncture to obtain exact pathological results.All patients completed whole body bone imaging and completed prostate puncture for exact pathological results.To collect the patients’ clinical data,including age,hemoglobin,blood calcium,serum alkaline phosphatase,total prostate specific antigen(TPSA),free prostate specific antigen(FPSA),F/t Tvalue,Gleason score.Chi-square test,Rank sum test and Logistic regression analysis were used to describe the relationship between the above clinical indexes and bone metastasis in prostate cancer.Results:This study included 148 prostate cancer patients,Including 83 cases of bone metastases,and 65 cases without bone metastasis.(1)The age,alkaline phosphatase and free prostate specific antigen values of the patients with bone metastasis were higher than those of the group without bone metastasis(P<0.05),Hemoglobin level and blood calcium level in bone metastasis group were lower than those in bone metastasis group(P<0.05),There was no significant difference in F/T between the two groups(P>0.05).(2)The value of Gleason score and TPSA were significantly different between the two groups(P<0.05).(3)Logistic regression analysis showed that age was a risk factor for bone metastasis in prostate cancer(P=0.036,OR=1.129,95%CI 1.008-1.264).Alkaline phosphatase values are a risk factor for bone metastasis in prostate cancer(P=0.000,OR=1.089,95% CI 1.044-1.136).Gleason score is a risk factor for bone metastases in prostate cancer,(P=0.004,OR=10.708,95%CI2.148-53.394).(4)Synthesize age,alkaline phosphatase and GS as a comprehensive model in Medcalc software.Draw ROC curve of three risk factors and the comprehensive model,Read the offline area of the four ROC curves.Comprehensive model(the measure of below-line area is 0.970)was better than age(the measure of below-line area is 0.693),alkaline phosphatase(the measure of below-line area is 0.905)and Gleason score(the measure of below-line area is 0.866)in predicting bone metastasis of prostate cancer.Conclusion:(1)Age,alkaline phosphatase values and Gleason score of prostate cancer patients are closely related to bone metastasis;(2)Synthesize age,alkaline phosphatase and GS as a comprehensive model.The effect of model predicting bone metastasis is better than age,alkaline phosphatase and Gleason score.When the patient is over 70 years old,alkaline phosphatase is more than84.5U/L,Gleason score is more than 7,the risk of bone metastasis is high and should attract sufficient attention.
