BackgroundSubretinal fibrosis scar in the macular region is one of the important reasons for vision loss in patients with PCV.Therefore,to explore the related risk factors of macular subretinal fibrosis scar in patients with PCV is of great significance to evaluate the visual prognosis of patients with PCV.ObjectiveObjective to compare the difference in the incidence of subretinal fibrosis scar in the macular area of patients with PCV under different baseline conditions. In order to explore the relevant influencing factors of subretinal fibrosis scar in PCV patients under real world conditions. In addition, the patients with AMD-CNV treated in the same period were collected for comparison,so as to understand the similarities and differences of subretinal fibrosis scar between PCV and AMD-CNV.MethodsFrom January 2015 to June 2020,110 patients(112 eyes)with PCV and 47 patients(51 eyes)with AMD-CNV who were initially diagnosed and treated and followed-up in our hospital were reviewed.General information of patients at baseline, such as age, gender, etc.as well as imaging characteristics of patients was collected.Patients received anti-VEGF therapy and/or photodynamic therapy and were followed up for no less than 3 months. To compare the influence of different factors on subretinal fibrosis scar in macular area in PCV and AMD-CNV patients, and to compare the difference of subretinal fibrosis scar between PCV and AMD-CNV patients.ResultsIn the last follow-up,14 eyes with PCV(12.5%)and 13 eyes with AMD-CNV(25.5%)had subretinal fibrosis scars. The best corrected visual acuity was significantly reduced in PCV(P=0.023)and AMD-CNV(P=0.001)patients with subretinal fibrosis scarring compared with those without.Univariate analysis showed that subretinal hyperreflective material(SHRM)(P=0.018)and follow-up time(P=0.016)had a significant influence on the occurrence of subretinal fibrosis scar in macular area of PCV patients.Binary Logistic regression analysis showed that SHRM was still statistically significant(P=0.035).In addition, there were significant differences in risk between patients with PCV and AMD-CNV(P=0.035)and between patients with PCV and typical CNV(P=0.028).There was no difference in the risk of subretinal fibrosis scarring between PCV and occult CNV patients(P=0.220).ConclusionsSHRM is an independent risk factor for subretinal fibrosis scar in PCV patients. In addition, the differences in the occurrence of subretinal fibrosis scars between PCV and different CNV types indicate that the damage of RPE layer may play an important role in the occurrence of subretinal fibrosis scars.
