Objective:1.To make a thorough inquiry in the different grades of the FAR(Fibrinogen/albumin ratio)and CAR(C-reactive protein/albumin ratio)in different grades of patients who suffered CAD.2.To research the relativity between FAR and CAR and the serious situation of patients who suffered from coronary heart disease.3.Explore the value of RAR and CAR in predicting severe coronary artery disease in patients with coronary heart disease.Methods:202 patients falling in coronary artery disease diagnosed by coronary arteriography from 2018-07 to 2020-07 were collected retrospectively in the Department of Cardiology of the Hospital,and their medical history,general clinical data,FIB,CRP,ALB,ALT,TBi L,Scr,LDL-C,HDL-C,CHO and TG were collected.FIB / ALB,CRP / ALB ratios were seriously calculated.Standard for the results of coronary angiography,assessment of severity of coronary artery disease using Gensini scoring criteria.Group the patients into three groups: severe lesion group(score ≥60),moderate lesion group(score≥30~<60)and mild coronary lesion group(score <30).The patients were divided into acute coronary syndrome group and stable coronary heart disease group according to clinical classification.The general clinical data,serum indicators,FAR and CAR of different groups were contrasted,and the pertinence between the ratios and the Gensini score was analyzed,the SPSS26.0 software performed all data analysis.Results:1.Comparison of basic clinical data: this study total included 202 patients,139 men,63 women,Average age(62.15±10.55)years.(1)Divide the Gensini score by 30 and 60,Based on Gensini integral,All patients were divided into mild coronary lesions(n=62,30.7%,moderate coronary lesion group(n=69%),34.2%,severe coronary lesions(n=71%),35.1%);There was a significant difference in smoking history between the three groups,the proportion of smoking patients in severe group and moderate group was larger than that in mild group,the difference was statistically significant,P<0.05,but there was no statistical difference in sex,age,drinking history,diabetes history,hypertension history,P>0.05.The FIB,CRP,CAR,FAR,TBIL,ALT and ALB level of patients in each group was statistically different,p<0.05,the FIB,CRP,CAR,FAR of patients in severe and moderate group was higher than that in mild group,the ALB level of patients in severe group was lower than that in mild group,and the ALT level was higher than that in mild group,the difference was statistically significant,p<0.05;There was no notable statistical distinction in Scr,LDL-C,HDL-C,CHO,TG among patients in groups.(2)Subgroup comparison of severe lesions:Non-heavy lesion group(n=131,64.9%),severe lesion group(n=71,35.1%).The results showed that there was significant difference in CRP,FIB,ALB,CAR,FAR,ALT,LDL-C between the two groups,P < 0.05,but no difference in age,sex,history of hypertension and diabetes,history of smoking and drinking,TBIL,Scr,HDL-C,CHO,TGg,P>0.05.(3)The clinical classification was divided into acute coronary syndrome group(n=76,37.6%)and stable coronary heart disease group(n=126,62.4%),there was a significant difference in CRP,CAR,ALT,LDL-C,CHO between the two groups,p<0.05.Compared with the stable coronary heart disease group,the CRP,CAR,ALT,LDL-C,CHO,and Gensini socre level of patients with acute coronary syndrome group was higher,but there was no significant difference in age,sex,history of hypertension and Results:diabetes,history of smoking and drinking,FIB、ALB、 FAR、TBIL、 Scr、HDL-C、TG,p>0.05.2.Correlation Analysis between each factor and Gensini score:There was a positive correlation between FIB 、 CRP 、 CAR 、 FAR 、 ALT and Gensini score,P<0.01,indicating that these indexes were positively correlated with the degree of coronary artery disease;ALB and Gensini score were not relevant with the levels of pathological changes of coronary artery disease(p<0.01),indicating that the correlation between FAR、CAR and coronary artery disease was stronger than that of single serological index;smoking history,TBi L and coronary artery disease were not correlated(p>0.01).3.Logistic regression analysis,(1)Whether acute coronary syndrome is a dependent variable,ALT(OR=1.019,p=0.009)and LDL-C(OR 2.706,p=0.036)are risk factors for acute coronary syndrome in patients with coronary heart disease;(2)With or without severe coronary lesions,FAR(OR=1.167,p=0.032)and CAR(OR=1.008,p=0.012)and(OR=1.507,p=0.026)are risk factors for severe coronary artery disease.4.The ROC curve of the severe-group patients diagnosed by each factor:the area under the curve of FAR was 0.803,the best cut-off value was8.28%,the sensitivity was 0.843,the specificity was 0.643.The under the curve of CAR was 0.873,the best cut-off value was 29.28%,the sensitivity was 0.914,the specific was 0.770.The area under the curve of LDL-C was0.586,the best cut-off value was 2.35mmol/l,the sensitivity was 0.516,the specificity was 0.700.The area,specificity and sensitivity of FAR and CAR curves were LDL-C excellent.Conclusion:1.The lesions of Coronary artery more serious and the FAR、CAR level of patients were higher.2.FAR and CAR were positively correlated with the levels of pathological changes of coronary artery disease.3.FAR and CAR are the risk factors of the severe pathological changes of coronary atherosclerosis in patients who suffered from coronary heart disease.
