Objective:Among all malignant tumors, liver malignant tumors have an significant role in the medical field from therapy to prognosis. The treatment of liver tumor has always been an important subject for clinicians to research and investigate. In the process of surgical resection of patients with primary liver cancer,it is not only necessary to strictly control the surgical process, but also the prevention and treatment of postoperative complications of the patient is the top priority. In the prognosis of patients with primary liver cancer, the emergence of complications is a very difficult problem. If complications occur after surgery, the mild ones will affect the patient’s postoperative recovery and increase the day of patients in the hospital, and the severe ones will cause the patient to undergo a second operation or even die.In order to avoid as much as possible the postoperative complications of patients with primary liver cancer, first of all, during the operation, the operation should be performed carefully and gently in accordance with the operation specifications to minimize the side damage caused by the operation. It is also important to accurate assessment of the patient’s liver function before surgery. In the evaluation of liver function of patients with primary liver cancer,Child-Pugh score has been used clinically, but Child-Pugh score has certain defects in the evaluation of liver function of patients with primary liver cancer. Therefore, whether to use Child-Pugh score for patients with primary liver cancer has always been controversial. Therefore,in recent years, some scholars have proposed the use of ALBI score to evaluate liver function in patients after operation.ALBI score is a clinical calculation index proposed in recent years based on two blood indicators, albumin and bilirubin levels. On the one hand, calculation data are easy to obtain, and on the other hand, subjective evaluation such as ascites is excluded. As the ALBI score is a newly proposed scoring system in recent years, there are few studies on the score. Therefore, this article collects and analyzes the data of patients undergoing primary liver cancer resection in the Department of Hepatobiliary Surgery,Dalian Friendship Hospital,to understand the factors related to complications after hepatectomy, and to study the impact of ALBI score on the reasons for receiving surgical treatment. The predictive ability of complications after surgery in patients with HCC.Methods:Use retrospective analysis, the case system was used to query the relevant clinical imformation of patients with primary liver cancer admitted to Dalian Friendship Hospital from August 2018 to January 2021.After the collected data are summarized and sorted out,the statistical software is used for further analysis. The patients included in the study were determined according to the criteria of Diagnosis through relevant guidelines and all included patients had undergone radical hepatocarcinoma resection and were determined to accept according to relevant research prompts, choose bleeding, infection, bile leakage, hepatic encephalopathy, and lung infection as the criteria for judging whether postoperative patients have complications Collect the relevant clinical data of the patient, and calculate the preoperative ALBI score and classification of the patient based on the preoperative clinical data.Use relevant statistical methods to analyze the obtained data.obtain the danger factors of complications after sugery in patients with primary liver cancer, and the predictive ability of ALBI score for complications.Result:From August 2018 to January 2021,a total of 48 patients with confirmed HCC were admitted through the Department of Hepatobiliary Surgery,Dalian Friendship Hospital, and all patients underwent open hepatectomy.According to the clinical data of 48 patients,23 patients were classified as ALBI I,14 patients were ALBI II,and the remaining 11 patients were ALBI III.All the above 48 patients successfully completed laparotomy for primary liver cancer.A total of 31 patients gradually relieved postoperatively, and various clinical indicators returned to normal.Complications occurred in 17 patients, and these 17 patients were classified by ALBI.2 patients were grade I,5 patients were grade II,and 10 patients were grade III.After statistical analysis of relevant indicators, the main risk factor for complications after hepatectomy is higher ALBI grade.Compared with patients with ALBI grade 3,ALBI grade 2(OR=0.010,95%CI0.0002-0.429,P=0.031),ALBI grade 1(OR=0.027,95% CI 0.001-0.945,P:0.046).The above analysis shows that the ALBI score has certain value in predicting the occurrence of complications in patients undergoing liver resection. Conclusion:The finest choice for the therapy of patients with HCC is still liver cancer resection. The preoperative ALBI score has a certain ability to predict whether there will be complications after surgery, that is,the higher the ALBI score, the higher the possibility of postoperative complications, and the severity of complications increases with the increase in ALBI score.Albi score is a simpler liver function evaluation system for obtaining score data.It only needs two blood biochemical test results to be calculated at the bedside. With good objective predictability and easy-to-use characteristics,in the clincial,it is an effective choice for modern clinical applications and is deserve to extensive promotion... |
