[Objective]To explore the evaluation value of preoperative nutritional status of patients with primary liver cancer and the effect of poor skeletal muscle quality on postoperative prognosis, and to analyze the predictions of sarcopenia on postoperative complications.[Methods]A retrospective analysis was conducted on110 patients with primary liver cancer treated from 2013 July to 2018 June in Quanzhou First Hospital affiliated to Fujian Medical University.According to the measurement of the area(cm2)and density(Hounsfield unit)of the patient’s psoas major in the skeletal muscle at the level of lumbar vertebrae(L3)on CT.Gender specific quartiles were then generated and the lowest 25% within the approach met criteria for sarcopenia. The subjects were divided into observer group(27cases,sarcopenia patients)and control group(83 cases,non-sarcopenia patients).Collecting data of patients including basic clinical data, postoperative complications, clinical outcomes and so on.SPSS 25.0statistical software was used to analyze the correlation between sarcopenia and postoperative prognosis of patients with primary liver cancer.[Results]Total of 110 patients with primary liver cancer were enrolled in this study. The analyzed data are as follows:(1)The median age of patients in the observer group was significantly higher than that in the control group(P<0.05).The patients in the observer group had lower serum albumin concentration and worse liver function than those in the control group(P<0.05);(2)The prognosis of the patients in the observer group was significantly poor(P<0.05).The patients in the observer group had higher postoperative complications;(3)The length of hospital stay and total hospitalization costs of patients in the observer group were significantly increased compared with those in the control group(P<0.05);the length of hospital stay after surgery in the observer group was longer than that in the control group.(4)Sarcopenia,the number of tumors, and the length of hospital stay after surgery are correlated with adverse outcomes in patients with liver cancer(P<0.05).Which are independent risk factors of surgical outcome for primary liver cancer.[Conclusions](1)Age,serum albumin concentration and child-Pugh grade are risk factors for sarcopenia in primary liver cancer.Improving preoperative serum albumin concentration and liver function are beneficial to reduce the occurrence of sarcopenia in patients with primary liver cancer.(2)Sarcopenia as measured with the Hounsfield Unit Average Calculation(HUAC),a value data that can be obtained from a preoperative CT scan,is a significant independent predictor for surgical outcomes.Sarcopenia, tumor numbers.and the length of hospital stay after surgery can increase incidence of postoperative complications.
