Objective:To investigate the value of color Doppler ultrasonography in evaluating hepatic venous blood flow spectrum in healthy people.Method:A total of 150 volunteers who did not meet the inclusion criteria such as arrhythmia, fatty liver, and hypertension were randomly selected from the physical examination center of the First Hospital of Jilin University for hepatic vein Doppler ultrasonography, and the general data and physical examination information were statistically analyzed.Finally,96 healthy individuals(38 males and 58 females,aged18-60 years, mean age 30 years)were included.Healthy volunteers fasted for 8 – 12 hours. In this study, the 3-5MHZ abdominal convex array probe of the My Lab Touch ultrasound Doppler instrument was used to preserve the middle hepatic vein spectral images in the right intercostal space and subxiphoid process, respectively. In all subjects, the hepatic vein spectra at the end of quiet expiration, deep inspiration, and deep expiration were taken under the right intercostal space, and A wave,S wave,V wave, and D wave were measured, and the S/D ratio, pulsatility index VPI(A/S),and damping index DI(V/S)were calculated.IBMSPSS statistics22 was used for data analysis: quantitative data were described as mean ± standard deviation,one-way ANOVA test was used for comparison between multiple groups;qualitative data were described as frequency(constituent ratio),and 2 test was used for comparison;P < 0.05 was considered statistically significant.Results:1),There was no significant difference between gender, age, heart rate,BMI,systolic blood pressure, and diastolic blood pressure between different spectral types under quiet expiration in healthy individuals(P > 0.05)2)The spectral types of hepatic veins at the end of quiet expiration in healthy individuals were HV0 and HV1,mainly HV0,accounting for 83%,of which HV0 type could be divided into triphasic wave, quadriphasic wave V type, and quadriphasic wave C type, accounting for 67%,11%,and 5%,respectively;HV1 type could be divided into biphasic wave, accounting for 17% of the total spectrum.3),The parameters of triphasic waves in the hepatic vein spectrum of healthy individuals ranged from 15.91 ± 5.30 cm/s for A waves,32.28 ± 8.69 cm/s for S waves,25.94 ± 6.95 cm/s for D waves,1.26 ± 0.23 for S/D,14.55 ± 6.19 cm/s for V waves,0.50 ± 0.13 for VPI,and 0.45 ± 0.14 for DI.The range of each index of four-phase wave(V-mode)in the hepatic vein spectrum of healthy individuals was17.68 ± 5.88 cm/s for A-wave,31.67 ± 7.27 cm/s for S-wave,18.71 ± 4.59 cm/s for D-wave,1.73 ± 0.38 for S/D,12.18 ± 5.83 cm/s for V-wave,0.56 ± 0.12 for VPI,and0.37 ± 0.12 for DI.The range of each index of four-phase wave(type C)in the hepatic vein spectrum of healthy individuals was 16.12 ± 1.59 cm/s for A wave,38.83 ± 2.99cm/s for S wave,29.92 ± 9.65 cm/s for D wave,1.45 ± 0.64 for S/D,11.47 ± 1.72cm/s for V wave,0.42 ± 0.053 for VPI,and 0.30 ± 0.052 for DI.The range of biphasic waves in the hepatic vein spectrum of healthy individuals was 29.76 ± 6.72 cm/s for S wave,26.43 ± 6.96 cm/s for D wave,1.15 ± 0.12 for S/D,17.82 ± 7.56 cm/s for V wave, and 0.59 ± 0.16 for DI.4),There was a significant difference between hepatic vein spectrum types under different respiratory states(P < 0.05)5),There was no significant difference in the type of hepatic vein spectrum between different measurement sites(P > 0.05)Conclusion:1.The hepatic vein spectrum of healthy people is mainly normal triphasic waves, which can also present as normal variants such as quadriphasic wave V type, quadriphasic wave C type, biphasic wave, and monophasic wave.2.Respiratory motion affects waveform changes, and deep inspiration increases the proportion of biphasic and monophasic waves in the hepatic vein spectrum.3.Different measurement sites(subxiphoid and right intercostal space)have no effect on the spectral morphology of the middle hepatic vein.
