| Objective: The purpose of this study is to observe and study the effect of Wenshen Tiaoshen recipe on intestinal probiotic flora(bifidobacteria,lactic acid bacteria)and renal function in CKD 2～3a stage of Yang deficiency,and to explore the clinical therapeutic mechanism of Wenshen Tiaobody Formula on Yang deficiency CKD 2～3a based on the theory of "intestinal-renal axis",so as to provide theoretical and practical basis for clinical application of Wenshen Tiaoshen recipe in the early and middle stage of chronic kidney disease.Methods: A total of 35 patients who met the inclusion criteria were selected and included in the clinical study by self-control method.On the basis of routine treatment of western medicine,the subjects were treated with Wenshen Tiaoshen recipe(traditional Chinese medicine non-decoction granule)for 4 weeks.The changes and correlation of total curative effect,content of intestinal probiotics(bifidobacteria,lactic acid bacteria),TCM physique transformation score,laboratory index,safety index and their correlation after treatment were statistically analyzed,and the results were discussed.Results: 1.Basic data: a total of 35 patients met the inclusion criteria,4 patients were excluded,and finally 31 patients completed this study,including 20 female patients and 11 male patients,the average age was 43.16 ±7.54 years old,the age range was 31-58 years old,the average course of disease was 26.16 ±4.11 months,the range of disease course was18-33 months,the average body weight was 67.48 ±12.48 kg,the range of body weight was 46-94 kg.According to the disease classification,there were 20 cases in CKD G2 stage and 11 cases in CKD G3 a stage.2.Total curative effect: After treatment,the clinical efficacy was evaluated in 8patients with obvious effect,14 patients with effective effect,5 patients with stable condition,4 patients with no effect,and the total effective rate was 70.97%.3.TCM physique transformation score: after treatment,the patients’ Yang deficiency physique transformation score was significantly lower than before,by paired sample t-test,P < 0.05,suggesting that the difference before and after treatment was statistically significant.4.Laboratory indexes: After treatment,the contents of intestinal probiotics(bifidobacteria,lactobacillus),24-hour urinary protein and serum creatinine of patients showed statistical significance between before and after clinical treatment(P<0.05P<0.01).5.In terms of safety indicators,all the subjects had no obvious adverse reactions and abnormal changes related to the study throughout the study,so the safety of this study should be affirmed.Conclusion: The prescription of warming kidney and regulating body has a good clinical effect in the treatment of G2 and G3 a patients with chronic kidney disease with Yang deficiency constitution.The serum creatinine,glomerular filtration rate,intestinal probiotic content,the conversion score of yang deficiency in traditional Chinese medicine and the quantity of 24-hour urinary protein were all improved compared with those before treatment,indicating that Wenshen Tiaoshen recipe can effectively adjust the intestinal microecology of patients with chronic kidney disease in G2 and G3 a stage,improve their renal function and physical condition,reduce the interaction between intestinal microecological disorder and chronic kidney disease,and then effectively delay the development of CKD.The prescription of warming kidney and regulating body is worthy of further research and application in clinic. |
