| Adenovirus pneumonia in children is one of the common types of pneumonia in children.Fever is often as high as 39℃ or more, manifested as lingering fever, while there can be cough, wheezing and other symptoms.Due to the lack of effective antiviral drugs,it is easy to develop severe.Severe pulmonary complications and sequelae were more common, and the mortality rate was higher. Objective:To study the risk factors of severe adenovirus pneumonia in children, evaluate the serious complications and search for effective drugs to reduce the serious complications. Methods:A total of 80 children with adenovirus pneumonia who were admitted to the Pediatric Respiratory Department and the Pediatric ICU Department of the First Hospital of Bethune,Jilin University from August 2018 to January 2021 were included in the study. The children met the diagnostic recommendations of the 8th edition of Zhufu Tong Practical Pediatrics.Using retrospective study method, statistical all into the adenovirus pneumonia in children with basis of gender, age, disease history, pneumonia, fever, breathing in the course of the situation, difficulty breathing,CK-MB level, pleural effusion, respiratory failure situation, merge infection, treatment situation and situation of serious complications such as content, using statistical analysis may be risk factors for severe adenovirus pneumonia in children under different drug treatment, evaluation of serious complications.Results:(1)Single factor analysis:There was no significant difference in genders、age ≤2 years and >2 years、every grade of thermal spike、every CK-MB level、multiple infections or not、complication of choamydiae infection or not、complication of fungal infection or not、and complication of respiratory syncytial virus infection or not among all groups. There were statistically significant differences in thermal course ≥7 days and thermal course < 7 days、wheething or not、dyspnea or not, pleural effusion or not、complication of mycoplasma infection or not、complication of bacterial infection or notamong all groups.(2)Multivariate logistic regression analysis:Logistic regression analysis was performed on factors with statistically significant differences in univariate analysis(thermal course ≥7days、wheething、dyspnea,pleural effusion、complication of mycoplasma infection、complication of bacterial infection),the results indicated that dyspnea, pleural effusion and mycoplasma infection had statistical significance between the severe group and the non-severe group.(3)Evaluation of serious complications under different drug treatments: there was no statistical significance in the application of antimicrobial agents, azithromycin, arabidosine, budesonide atomization, interferon atomization, intravenous application of glucocorticoids, human immunoglobulin and other drugs among the groups.Conclusions:(1)Dyspnea,pleural effusion and mycoplasma infection are independent risk factors for severe adenovirus pneumonia in children.(2)In severe adenovirus pneumonia in children, bacterial drug resistance, azithromycin, vidarabine atomization, budesonide atomization, interferon, intravenous application of glucocorticoid, human immunoglobulin drugs such as does not reduce pleural effusion, respiratory failure, lung consolidation, toxic encephalopathy, circulation failure, atelectasis and other serious complications. |
