Clinical Study On The Method Of Invigorating The Spleen And Supporting Yang In The Treatment Of Obese Pre-Diabetes Mellitus (Spleen Deficiency And Phlegm-Dampness Syndrome)

Objective: This study adopts the method of invigorating the spleen and supporting Yang to treat spleen deficiency and phlegm-dampness syndrome in the prediabetic stage of obese type based on the theory of "Yifusan ointment".In addition,it intends to verify the clinical efficacy and safety of the method of invigorating the spleen and supporting yang,warming phlegm-dampness and guiding pathogen out in the treatment of obese Pre-diabetes mellitus so as to provide new ideas and methods for the treatment of obese Pre-diabetes mellitus.Methods: 72 patients with spleen deficiency and phlegm-dampness syndrome in pre-diabetes were randomly divided into treatment group(n = 36)and control group(n =36).The two groups were treated on the basis of strict diet and exercise therapy.In addition,the treatment group was treated with Lizhong decoction and the control group was treated with acarbose tablets.The changes of TCM symptom score,fasting blood glucose(FPG),blood glucose(2h PG),glycosylated hemoglobin(Hb A1c),fasting serum insulin(FINS),insulin resistance index(HOMA-IR),blood lipid index(TG,LDL-C),waist-to-hip ratio(WHR)and body mass index(BMI)were observed before and after treatment.4 weeks was a course of treatment,and 3 courses were observed continuously.Results: Both the treatment group and the control group were effective in reducing blood glucose level and physicochemical indexes after treatment(p < 0.05).The effect of reducing FPG in the treatment group was better than that in the control group(p﹤0.05).However,there was no significant difference in reducing 2h PG and Hb A1 c between the treatment group and the control group(p > 0.05).In terms of TG,the curative effect of the treatment group was better than that of the control group(p < 0.05).However,there was no significant difference in LDL-C between the treatment group and the control group(p >0.05).The treatment group was superior to the control group in improving FINS,HOMA-IR,WHR and BMI.The improvement of TCM symptoms in the treatment group was significantly better than that in the control group(p < 0.05).Conclusion: Under the guidance of the theory of "Yifusan ointment",the method of invigorating the spleen and supporting Yang is effective in the treatment of Pre-diabetes mellitus.It can effectively regulate glucose and lipid metabolism,improve insulin resistance and improve TCM symptoms,lose weight,and reduce waist-to-hip ratio.Moreover,there is no adverse reactions,and no damage to liver and kidney function and hypoglycemia in the course of treatment.Generally speaking,it is safe and effective in clinic and is worth further popularizing and applying.
