Objective: With the increasing number of elderly population in China,China’s medical care and social security are greatly challenged,which has a great impact on people’s life style and family relations.In developed cities of China,the exploration and establishment of combined medical and nursing institutions is relatively successful,but the development mechanism of combined medical and nursing institutions in rural areas is not yet mature.Therefore,this paper takes a rural area of Dalian City as an example to study the ethical problems in the medical and nursing institutions in rural areas,and according to the actual situation,puts forward countermeasures and suggestions for the ethical problems found,in an effort to explore its ethical value,enrich its ethical implications,and provide a basis for the system improvement of China’s rural medical and nursing institutions.Methods: After mastering the relevant basic theoretical knowledge through literature research,the elderly over 60 years old in the rural medical and nursing institutions and surrounding villages and towns in Dalian were surveyed by questionnaire method.A total of 130 questionnaires were sent out and 126 questionnaires were collected in this survey,with the recovery rate was 96.92%.This paper conducted in-depth interviews with the elderly and the staff in the rural medical and nursing institutions,summarized the ethical problems in the rural medical and nursing institutions,and put forward suggestions and countermeasures according to the existing problems and needs in the rural elderly care.Results: The quality of life of the elderly in rural areas is affected by three factors,including:(1)Family factors,the elderly are more likely to suffer from psychological and physical problems due to the absence of their children.(2)Economic factors,the elderly in rural areas are financially over-dependent on their children.(3)Medical factors,the medical conditions in rural areas are poor,and the elderly in rural areas have a negative attitude towards medical treatment.In rural areas,the ethical problems existing in the institutions combining medical care and nursing care are as follows:(1)The combination of medical and nursing institutions is not deep enough and the public welfare is low.(2)The professional ethics of institutional nursing staff is low.(3)Institutions do not pay enough attention to the mental health of older people.(4)Humanistic care for the elderly in institutions is insufficient.(5)Institutions neglect to maintain the ethical environment for the elderly.Research data,it is found that the establishment of the medical and nursing institutions in the rural areas of Dalian has not solved the dilemma and ethical problems of the elderly in the rural areas,and has not met the needs of the elderly residents in the rural areas.Countermeasures and Suggestions:(1)Strengthen the awareness of government responsibility ethics,and protect the public welfare attributes of institutions.(2)The training mechanism of humanistic care should be established to improve the ethical quality of nursing staff.(3)Pay attention to the psychological problems of the elderly and build a social support system.(4)Shape the humanistic care environment of institutions,deepen the concept of old-age service.(5)Strengthen the popularization of legal knowledge,attach importance to the moral education of the elderly.
