| Objective: Taking the patients with POP in Changchun area as the investigation object, the risk factors of the incidence of POP in Changchun area were analyzed.According to the above data, the distribution of TCM syndromes was analyzed. Finally,it provides objective basis for early clinical intervention of primary osteoporosis. Methods: Using multi-stage cluster stratified random sampling method,1000 permanent middle-aged and elderly residents over 40 years old were selected, and then they were asked to fill in the questionnaire of risk factors of primary osteoporosis and the diagnostic criteria of TCM syndrome type.A total of 485 patients diagnosed with POP were selected as the case group, and patients who did not meet the POP diagnosis during the same period were selected as the control group. For the factors related to the incidence of POP,univariate analysis and multivariate Logistic regression model were used for case-control analysis.SPSS22.0 software database was used for statistical analysis to explore the correlation between related risk factors and primary osteoporosis.Descriptive statistics of TCM syndromes of 485 POP patients were carried out according to the questionnaire, and then the distribution of TCM syndromes was studied. Then, the relationship between the related factors and the TCM syndromes was analyzed by using the corresponding single cause analysis.Results:1.A total of 1000 questionnaires were sent out,of which 30 were interrupted in the process of questionnaire investigation and could not be completed, with an effective rate of97%.Among them, there were 485 cases in the case group and 485 cases in the control group(undiagnosed with primary osteoporosis). In this valid data, there are 396 males and574 females, accounting for 40.8% and 59.2% of the total number respectively.2.Logistic regression analysis was applied to the data collected from the questionnaire to obtain general results.Among them, age and gender(OR=6.653),BMI(OR=0.455),(OR=1.097),and family history of brittle fracture(OR=1.639)were risk factors for osteoporosis. The consumption of dairy products(OR=0.302),consumption of soy products(OR=0.498)and exercise(OR=0.238)were the protective factors for osteoporosis, while the others had no correlation with the incidence of POP.3.In this study, the syndrome types of 485 POP patients with different TCM syndromes were distributed as follows: The total number of patients with deficiency of liver and kidney Yin was 117(24.1%),which accounted for the highest proportion, followed by 99 patients with deficiency of spleen and kidney Yang(20.4%),84 patients with stagnation of blood stasis and qi(17.3%),96 patients with deficiency of kidney Yang(14.2%),60 patients with deficiency of kidney and blood stasis(12.4%),and 56 patients with weakness of spleen and stomach(11.5%).4.There was no significant difference in sports status, consumption of soy products and family history of fragile fracture among different TCM syndrome types(P>0.05).There were statistically significant differences in menopausal age, gender, dairy consumption,BMI,age and other factors among female patients with different TCM syndromes(P<0.05).Conclusion:1.Gender,age,menopausal age, body mass index and family history of fracture were the risk factors for primary osteoporosis, while daily exercise, soy and dairy consumption were the protective factors for osteoporosis.2.The distribution of TCM syndromes in the case group of 485 patients with primary osteoporosis was different.Liver and kidney Yin deficiency syndrome and spleen-kidney Yang deficiency syndrome were TCM syndromes that should be paid attention to in clinical treatment.3.Gender,age,menopausal age,BMI,dairy consumption and other factors affect the distribution of TCM syndromes of POP.The risk factors and protective factors of each TCM syndrome type are the same and have different emphasis. |
