ObjectiveThe expression levels of soluble human matrix cleavage 2(s ST2)and galactose lectin 3(Gal-3)in serum of patients with HFmr EF,HFr EF and HFp EF were analyzed and compared.To explore the clinical characteristics of heart failure with median ejection fraction and the diagnostic value of s ST2 and Gal-3 in patients with different types of heart failure.MethodsA total of 133 patients with heart failure were collected from the Department of Cardiology,the second affiliated Hospital of Shenyang Medical College from December 2019 to December 2020(HF group),including 43 cases of HFr EF,42 cases of HFRF and 42 cases of HFp EF48.According to the heart function classification,the patients with heart failure were divided into NYHA Ⅱ(n = 30),NYHA Ⅲ(n = 55)and NYHA IV(n = 48).In the same period,healthy people were selected as the control group(non-HF group).The general clinical data and laboratory examination of each group were collected,and the venous blood of elbow was collected after admission,and the serum concentrations of s ST2 and Gal-3 in each group were determined.The general clinical data,laboratory examination,s ST2 and Gal-3 serum concentrations of patients in each group were statistically analyzed,and the diagnostic significance and value of s ST2 and Gal-3 in patients with different types of heart failure were analyzed by(AUC)under the(ROC)curve of the subjects’ working characteristics.ResultsCompared with the baseline data of the four groups,the age of patients in HFmr EF and HFr EF group was younger than that in HFp EF group.The proportion of male patients with HFr EF,HFmr EF and HFp EF was 76.7%,61.9% and 47.9%,respectively.The male proportion of HFr EF and HFmr EF was higher than that of HFp EF group.In terms of medical history,the proportion of old myocardial infarction in patients with HFr EF,HFmr EF and HFp EF was 48.8%,33.3% and 2.1%,respectively.The proportion of history of atrial fibrillation was 14%,33.3% and 35.4%,respectively.The proportion of hypertension history was 69.8%,64.3% and 68.8%,respectively.The proportion of diabetes history was 51.2%,40.5% and 31.3%,respectively.In terms of medical history,HFr EF and HFmr EF groups suffered from old myocardial infarction and glycosuria more frequently than those in HFp EF group.The history of atrial fibrillation in patients with HFmr EF and HFp EF was more than that in patients with HFr EF.There were significant differences in age,NT-pro BNP and s ST2 between HFr EF and HFmr EF group.There were significant differences in NT-pro BNP and s ST2 between HFr EF and2 HFp EF group.There were statistical differences in NT-pro BNP and s ST2 between HFmr EF and HFp EF group.The higher the grade of cardiac function,the higher the serum concentration of NTpro BNP,s ST2 and Gal-3.By ROC analysis,compared with the control group,the area under the ROC curve of s ST2 and Gal-3 of HFmr EF was0.721 and 0.785,and the boundary value was 20.65 ng / ml and 18.43 pg / ml,respectively.The area under the ROC curve of NTpro-BNP combined with s ST2 and Gal-3 was 0.892 and 0.898 respectively.Compared with the control group,the areas under the ROC curve of s ST2 and Gal-3 in HFp EF were 0.706 and 0.744,and the boundary values were 21.4 ng L and 18.35pg/ml,respectively.The area under the ROC curve of NTpro-BNP combined with s ST2 and Gal-3 was 0.885 and 0.892 respectively.Compared with the control group,the area under the ROC curve of s ST2 and Gal-3 in HFr EF was 0.840 and 0.749,and the cutoff value was 26.89 ng / L,18.87pg/ml,respectively.The area under the ROC curve of NTpro-BNP combined with s ST2 and Gal-3 was 0.981 and 0.967 respectively.ConclusionsAccording to the analysis of cardiac function grade,the serum concentration of s ST2 and Gal-3 was positively correlated with the grade of cardiac function;by ROC analysis,s ST2 and Gal-3 had certain value in the diagnosis of heart failure,and the combination of NTpro BNP and s ST2 or Gal-3,could improve the diagnosis of heart failure;compared with the control group,the diagnostic value of Gal-3 in patients with HFmr EF and HFp EF was higher than that in patients with heart failure.The diagnostic value of s ST2 in patients with HFr EF is higher than that of Gal-3.
