| Objective:The purpose of this study is to observe the clinical efficacy of acupuncture combined with Fuyang cupping warm curettage in the treatment of acute periarthritis of shoulder(wind cold dampness type).The simple acupuncture group was set as the control group,so as to verify the effect of the former on the treatment of this disease,so as to provide a more safe and effective treatment scheme for the clinical treatment of this disease,so that the clinical work can be better carried out.Methods: Selected patients with acute rheumatic-cold-damp periarthritis in the Department of Acupuncture and Moxibustion,Guanggu District,Hubei Provincial Hospital of Traditional Chinese Medicine from January 2019 to January 2020.On the premise of the patient’s willingness,cases were screened according to the established diagnostic criteria,inclusion criteria and exclusion criteria,and a total of 60 patients were included.In order to ensure that the results of the experimental study are more representative,all volunteers were randomly divided into the acupuncture group and the needle cupping group,with an average score of 30 patients in both groups.The 60 subjects in the study were divided into groups in a random way,and the final result was 30 people in the needle pot group and 30 people in the acupuncture group.The treatment methods of the combined acupuncture and cupping group: 1.Acupuncture treatment,the main acupoints are Jianyu,Jianliao,Jianzhen,Jianqian,Ashi,Yanglingquan,Tiaokou-through-chengshan,and the matching points are Hand-YANGMING Model with Sanjian,Hand-SHAOYANG model with Zhongzhu,Hand-TAIYANG model with Houxi,Hand-TAIYING model with Lieque.Two days treatment,three times a week,a course of 2 weeks,a total of four weeks,namely two courses2.Fu yang pot warm curettage treatment focuses on the pain and tenderness of the patient.The pain and tenderness often occur: the front of the shoulder,the long head tendon of the biceps,the subacromial,the deltoid stop,the shoulder and neck,Shoulder blades,behind shoulders.Find out the pain and tenderness of the patient,determine the meridian where the pain and tenderness are,and perform the operation along the meridian.The surgeon holds the warm Fu yang jar,and scrapes at a 45° angle between the warm ceramic edge and the skin.The redness of the skin is used as the degree,and Sha is not forced.The treatment process was the same as that of the acupuncture group.Treatment method of acupuncture group: acupoint selection and treatment courses were the same as those in the acupuncture cupping group.The VAS score,CMS score and overall curative effect of the two groups before treatment,4 weeks after treatment and 8 weeks after treatment were statistically analyzed,so as to evaluate the short-term and long-term efficacy and safety of acupuncture combined with Fuyang cupping and warm scraping method and acupuncture in the treatment of acute periarthritis of shoulder(wind cold dampness type).Results:1.Completion of this study: among the 60 patients included in this study,1 case was dropped out and 1 case was removed.Finally,29 cases in both groups completed the study successfully.2.Comparison of pain improvement between the two groups: After completing two sessions of treatment,at the end of 4 weeks,the VAS scores of all patients in both groups were compared to those before treatment,P<0.01,indicating that the two treatment methods have excellent effects on alleviating the pain of frozen shoulder.Comparison of VAS between the two groups after four weeks of intervention,P<0.01,indicated that the acupuncture and cupping group was better than the traditional acupuncture group in improving the pain of frozen shoulder.The VAS score of the eighth group was compared with that of the baseline period,P < 0.01,indicating that the long-term effects of the two groups on alleviating frozen shoulder pain are worthy of recognition.Compared with before treatment,the VAS scores of the two groups at the 8th week were all P < 0.01,indicating that the long-term efficacy of the two groups in relieving the pain of periarthritis of shoulder was worthy of affirmation.The comparison between the two groups after 8 weeks of inclusion and 4 weeks of treatment showed that the acupuncture group(P < 0.05)and the acupuncture and cupping group(P < 0.01).The comparison of VAS scores between the two groups after 8 weeks of inclusion showed that the acupuncture and cupping group had more advantages than the acupuncture group in the long-term efficacy of relieving periarthritis of shoulder pain.3.Comparison of the improvement of shoulder joint function between the two groups:Comparison of 4-week treatment and baseline CMS,P<0.01,indicating that the two groups have significant effects on improving shoulder joint function of patients with frozen shoulder.Comparison of CMS after 4 weeks of intervention,P<0.05,there was a difference,indicating that the improvement of shoulder joint function of patients with frozen shoulder in the acupuncture-cupping group was better than that of the acupuncture group.After 8 weeks,the CMS scores of the two groups were compared with those before treatment and 4 weeks after treatment,P<0.01,indicating that the long-term recovery effect of the two groups on improving shoulder joint function in patients with frozen shoulder is worthy of recognition.After 8 weeks of treatment,P>0.05,indicating that the acupuncture group and the acupuncture group do not have obvious advantages in improving the long-term effect of shoulder joint function in patients with frozen shoulder.4.Analysis of the total effective rate: in this study,96.6%of the subjects in the acupuncture and cupping group achieved the curative effect in improving the symptoms of periarthritis of shoulder,while only 89% of the subjects in the acupuncture and cupping group achieved the curative effect.It can be seen that the overall curative effect of the acupuncture and cupping group was better.5.Safety evaluation: During the entire study process,there were no adverse events such as burns,skin allergies,needle fainting,faint moxibustion,faint scraping,and subcutaneous hematoma during the treatment.Conclusion:The short-term and long-term effects of the two groups on alleviating the pain of frozen shoulder are worthy of recognition,but the acupuncture group has more advantages.In improving shoulder joint function in patients with frozen shoulder,the short-term and long-term curative effects of the two groups are worthy of recognition.Comparing the two groups,from the short-term effect,the acupuncture and cupping group is better,but the long-term effect of the two groups is not much different.In terms of overall curative effect,the acupuncture group is 96.6% and the acupuncture group is 89%.The clinical curative effect of the acupuncture group is more advantageous.All in all,Acupuncture combined with Fuyang tank warm scraping is more effective than simple acupuncture.It can reduce the patient’s pain,improve the function of the shoulder joint,and the effect is better.Fuyang tank is simple,safe and effective to operate,and it is worthy of clinical application. |
