| Purpose:In the urinary system,the probability of tumors in different parts is different,and the incidence of bladder cancer is the highest in clinical work.Globally,studies have found that the overall incidence of bladder cancer is eleventh in systemic tumors,with men slightly higher and seventh,and women seventeenth.In China,men with bladder cancer rank seventh in the incidence of tumors in all parts of the body,while women with bladder cancer are ranked tenth.According to the National Cancer Registry in 2009,the bladder cancer mortality rate was 2.60/100,000,including 3.75/100,000 for men,1.24/100,000 for women,and 2.97:1 for men and women.According to statistics,after transurethral resection of bladder tumors,about 30%-60%of patients still have recurrence.However,some relapses are at risk for invasive bladder cancer at the grassroots level or local and distant organ tissues.The risk ratio is between 10%and 15%.The high incidence,mortality and recurrence rate of bladder cancer seriously affect the quality of human life.However,the pathogenesis of bladder cancer is not completely clear at this stage,so it is urgent to explore its pathogenesis.It has been reported that Tiam1 and CD44 expression products can be detected in most tumor tissues such as breast cancer,oral cancer,and gastrointestinal cancer,and their expression is statistically significant.The purpose of this experiment is to detect the expression of Tiam1 and CD44 in bladder cancer tissues,analyze whether they are related to the pathological parameters of bladder cancer,and explore the mechanism of tumor cell invasion and infiltration,in order to detect and diagnose bladder cancer early,and provide future clinical work.Theoretical support.Method:Immunohistochemical staining was used to detect the expression of Tiam1 and CD44 in 111 clinically diagnosed bladder cancer tumor tissues and 37 normal bladder tissues adjacent to the cancer,and retrospectively analyzed the differences between the expressions and clinical pathological parameters of bladder cancer(such as tumor size,TNM stage,pathological grade,etc.),and the correlation between the two expressions.Result:1.The positive expression rate of Tiam l in bladder cancer tissue was 76.6%(85/111),and the positive expression rate of Tiam1 in normal tissue adjacent to the control group was 24.3%(9/37).The expression of Tiam l in bladder cancer tissue and normal bladder tissue was significantly higher in the former and statistically significant.In the≤T1stage(except Tis)group,the positive expression rate was 70.1%(54/77),and the≥T2stage group was 91.2%(31/34).The positive expression rate was 90.6%(29/32)in the high-grade pathological grade group and 70.9%(56/79)in the low-grade highly differentiated group.The positive expression rate was71.1%(59/83)in the non-lymph node metastasis group and 92.9%(26/28)in the lymph node metastasis group.The positive expression rate was71.6%(58/81)in the non-vascular invasion group and 90.0%(27/30)in the vascular infiltration group.Different clinical stages of bladder cancer,pathological grade,lymph node metastasis and vascular invasion affect the expression of Tiam1 and have statistical significance;Tiam l has no significance in different expression levels of bladder cancer tumor size,gender,and age.2.The positive expression rate of CD44 in bladder cancer is 67.6%(75/111),and the positive expression rate of CD44 in normal tissues adjacent to the cancer is 18.9%(7/37).The expression of CD44 in bladder cancer tissues and normal tissues was significantly higher in the former and statistically significant.The positive expression rate in the group≤T1(except Tis)was 59.7%(46/77),and≥T2was 85.3%(29/34).The positive expression rate was 78.1%(25/32)in the high-grade group and 63.3%(50/79)in the low-grade highly differentiated group.In the non-lymph node metastasis group,the positive expression rate was 63.9%(53/83),and in the lymph node metastasis-positive group,it was 78.6%(22/28).The positive expression rate was 61.7%(50/81)in the non-vascular infiltration group and 83.3%(25/30)in the vascular infiltration group.Different clinical stages,pathological grades,lymph node metastasis and vascular invasion of bladder cancer affect the expression of CD44 and have statistical significance.The size,age,and gender of bladder cancer tumors had no effect on the expression of CD44.3.Statistical analysis was used to infer that the expressions of Tiam 1 and CD44 in bladder cancer may be positively correlated(correlation coefficient r=0.726,χ2=10.14,P<0.01).Conclusion:1.Tiam1 and CD44 are highly expressed in bladder cancer tissues,and may be positively correlated with clinical stage,pathological grade,lymph node metastasis and vascular invasion of bladder cancer.2.The expressions of Tiaml and CD44 in bladder cancer may be positively correlated,indicating that Tiam1 and CD44 may have synergistic effects in the occurrence and development of bladder cancer,and jointly promote tumor cell proliferation,local lymph nodes and vascular infiltration. |
