[OBJECYTIVE] To study relationship between expression of the bax, Bcl-2, CD44 and nm23 protein in transitional cell carcinoma (TCC) of the bladder and their occurrence, development and prognosis.[METHODS] Using the S-P immunohistochemical staining, it was detected that the expression of the bax, Bcl-2, CD44 and nm23 in 64 cases of TCC and 20 cases of normal bladder mucosa.[RESULTS] Of 64 cases of TCC, the expression of bax was 17.2% (11/64) but in 20 normal bladder mucosa it was90.0% (18/20) (P<0.05), and for bcl-2, it' s expression were 82.8% (53/64) and 20.0% (4/20) respectively (P<0.05). The positive rate of bax and bcl-2 had the trend of improvement with the development of grade but it hadn't the positive correlation (P>0.05). In non-invasions and depth-invasions of TCC, bax expression were 16.3% and 20.0% respectively (P>0.05), bcl-2 expression were 83.7% and 80.0% respectively (P>0.05). In non-recurrences or metastases and in recurrences or metastases of TCC, bax expression were 16.1% and 25.0% respectively (P>0.05), bcl-2 expression were 82.1% and 87.5% respectively (P>0.05). In 64cases of TCC, CD44 expression was 50.0% (32/64) but in normal bladder mucosa it was 5.0% (1/20) (P<0.05), for nm23 , it's expression were 76.6% (49/64) and 20.0% (4/20) respectively (P<0.05). The expression of CD44 protein had significant difference (P<0.05) between grade I, II and grade III and the expression of nm23 had significant difference (P<0.05) between grade I and grade II, III. The expression of CD44, nm23 in invasions had higher level than that in non-invasions. CD44 expression in metastases had higher level than that in non-metastases (P<0.05), while nm23 expression hadn't relation to its metastases (P>0.05).[CONCLUSION] The decreased expression of bax protein and the increased expression of bcl-2 protein play an important role in occurrence of bladder transitional cell carcinoma, and CD44 and nm23 protein also promote the occurrence of tumors, bax and bcl-2 protein had not related to the grade, stage and recurrence or metastasis of tumors. The expression of CD44 has related positively to the grade, stage and recurrence or metastasis, nm23 has related positively to the grade and stage of tumor, but has not related to recurrence or metastasis.
