Study On The Rule Of Medication In Different Age Groups And Combined Diseases Of Patients With Adenomyosis

Objective: Based on real-world clinical data of patients with adenomyosis(AM),the age of AM patients and the incidence of comorbid diseases are analyzed,and the rules of prescription of traditional Chinese medicine for different age groups and other diseases are analyzed.Taking the gynecological ultrasound measurement results as the evaluation standard of the curative effect,the characteristics of the changes of Chinese medicines used in the treatment of effective patients are analyzed to provide reference for the treatment of adenomyosis with Chinese medicine.Methods: 1.Establish database: collect the information of AM patients in gynecology outpatient department and ward of Affiliated Hospital of Shandong University of traditional Chinese medicine from 2012 to 2017,and collect the imaging information of adenomyosis patients in gynecology color Doppler room database,and integrate them to form the adenomyosis patients database.2.Data extraction and grouping:(1)extract the medication information of patients aged 20-55 from the database and group them according to their age groups;(2)group the patients according to the ultrasonic imaging diagnosis of the patients.Group A: AM with hysteromyoma alone;group B: AM with pelvic effusion alone;group C: AM with endometriosis alone;group D: AM with endometrial organic lesions alone.(3)Based on the changes of ultrasound imaging of gynecology as the evaluation criteria of the curative effect,we compared the size of the uterus before and after the treatment,combined with the changes of the size of the focus,the changes of blood flow signals in the focus to determine the curative effect,and extracted the effective patient information.3.Data analysis method:(1)frequency statistics: basic frequency statistics by Microsoft Excel.(2)Complex network analysis: realized by liquorice software.(3)Association Rules:by using multiscale backbone,the confidence is set to 0.96,and the number of compatible sides between nodes in the prescription is obtained.The number of sides between nodes is derived and the compatibility strength between two nodes is calculated according to the association rules.The higher the value is,the stronger the compatibility is.(4)RR value analysis method of traditional Chinese medicine: the epidemiological RR value is applied to the data mining of traditional Chinese medicine.By calculating the RR value of single traditional Chinese medicine and disease,we can get the traditional Chinese medicine which has a great correlation with the treatment of a disease,and then we can get the characteristic drug for clinical treatment of the disease.Results: A total of 3768 cases of adenomyosis were collected,involving 8986 prescriptions,392 traditional Chinese medicines,including 201242 pieces of traditional Chinese medicine information and 4322 pieces of ultrasonic imaging data.(1)Characteristics: the number of diagnosis of adenomyosis increased year by year from the age of 20,and decreased year by year to perimenopause.About 33.42% of the patients had hysteromyoma,25.54% with pelvic effusion,14.01% with endometriosis,mainly with hysteromyoma,endometriosis,pelvic effusion and endometrial lesions.(2)Age and the characteristics of drug use in the group of combined diseases: the common drugs of the core prescriptions of each group are vinegar Xiangfu,angelica,Ligusticum chuanxiong,vinegar yanhusuo and white peony.The patients aged 20-29 were mainly treated with blood tonics and blood activating analgesics.Patients aged 30 to 39 were mainly given blood tonic and Qi regulating drugs,while patients aged 40 to 49 were mainly given blood tonic drugs.The patients aged 50 to 55 years old used the most amount of Qi tonic drugs,and supplemented with yin tonic drugs and Gujing contractive urine tourniquet drugs.In the AM group with hysteromyoma alone,the RR value of Chinese medicine is more than1.5 and the frequency of use is more than the average.In the AM group with pelvic effusion alone,cinnamon,Salvia miltiorrhiza,Cimicifuga,etc.in the AM group with endometriosis alone,Curcuma acerifolia,Trigonopsis acerifolia and Scrophularia are the main ingredients.In the adenomyosis group with endometriosis alone,Schisandra chinensis,epimedium,Cimicifuga are the main ingredients And so on.(3)Based on the information of ultrasound,56 cases of effective patients were collected.Results: the traditional Chinese medicine taste bitter,sweet and pungent,with acid drugs.Cold and heat are the same,but the medicine is cold.The liver and spleen are the main channels of the medicine,followed by the kidney.Core prescription: vinegar Xiangfu,white peony,angelica,chuanxiong,oyster,vinegar yanhusuo,Puhuang,dangshen,motherwort,chuanniuxi.And statistics of high-frequency drugs and high-frequency drugs.Conclusion:(1)adenomyosis can occur from menarche to menopause,the lowest age is about 20 years old.In addition,some of them were associated with hysteromyoma,endometriosis,pelvic effusion and endometrial lesions.(2)The basic pathogenesis is blood stasis blocking Chong Ren,and the main therapeutic drugs are promoting blood circulation and removing blood stasis,regulating qi and relieving pain.The disease is mixed with deficiency and excess.With the increase of age,it changes from partial positive syndrome to partial deficiency syndrome.The patients with hysteromyoma need to stop bleeding and remove blood stasis.The patients with pelvic effusion can warm the meridians to dispel cold and dampness,or clear heat and clear water to dispel dampness.The patients with endometriosis need to break blood and eliminate syndrome to dispel stagnation.The patients with endometriosis need to clear heat and detoxify.(3)Based on the results of gynecological ultrasound,most of the patients with effective treatment are Qi and blood deficiency,qi stagnation and blood stasis,and the treatment mainly includes invigorating qi and nourishing blood,soothing liver and regulating qi,promoting blood circulation and removing stasis.In addition,the deficiency syndrome differentiation is to invigorate the spleen and kidney,nourish yin and warm yang,while the excess syndrome differentiation is to clear away heat and detoxification,dissipate phlegm and Dissipate Stasis.
