Clinical Study On The Treatment Of The Upper Heat And Lower Cold Syndrome Of Aqueous Tear Deficiency By The Combination Of Wumei Pill Plus Or Minus Prescription And Artificial Tears

Objective:Under the guidance of the core theory of "Four Scared Gist",writtened by Huang Yuan-yu,a renowned doctor in Qing dynasty:"earth pivot four-dimensional,qi circular harmony" according to the clinical manifestation of upper heat and lower cold syndrome of aqueous tear deficiency,formulated a therapeutic schedule that using the combination of Wumei Pill plus or minus prescription and artificial tears on treating the upper heat and lower cold syndrome of aqueous tear deficiency,and to explore the clinical curative effect.Methods:Selected 45 patients who met the inclusion criteria of this study were investigated with a randomized parallel controlled trial desigh,randomly divided into three groups with 15 patients in each groups.Joint group was treated by wumei pill plus or minus prescription and sodium hyaluronate eye drops;The Chinese medicine group was treated by wumei pill plus or minus prescription alone;The western medicine group was treated by sodium hyaluronate eye drops alone.To observe and compare the results of TCM sympton score,BUT,SIt,and corneal staining were observed before therapy and at post-treatment 30 days in the three groups,and were statistically processed to make an objective evaluation of the efficacy.Results:①Comparison of objective indexes of three groups of dry eyes.Before and after treatment,BUT,SIT,corneal staining were all improved in each group,with statistically significant differences(P<0.05).After treatment,there were statistically significant differences in SIT and corneal staining between the joint group and the western medicine group(P<0.05).In the aspect of BUT,the joint group had better efficacy than the other two groups(P<0.05).There was no statistically significant difference between the remaining groups(P<0.05).②Comparison of three groups of TCM symptom scor es.Before and after tr eatment in each group,there was no significant decrease in urination frequency score in the joint group,and the remaining symptoms were significantly improved(P<0.05).Except for no significant decrease in urination frequency and night sweats symptom score,the remaining symptoms were significantly improved in the Chinese medicine group gr oup(P<0.05).In the western medicine group,there was no statistically significant difference in the remaining symptoms except dryness,foreign body sensation,visual fatigue,r ed eyes,photophobia,burning sensation,dry mouth,bitter mouth and cold pain of abdominal distension(P>0.05).After treatment,there was no statistical significance in the difference of symptom scores between the joint group and the Chinese medicine group(P>0.05).Compared with the western medicine group,the score difference between the joint group and the western medicine group was statistical l y significant in photophobia,burni ng sensation,tinnitus,sores on the tongue and mouth,dry mou t h,bitter mouth,irritability,soreness and weakness of waist and knees,chills and cold limbs,loose stool(P<0.05).Compared wi th the western medicine group,the Chinese medicine group had statistically significant differences in dry mouth,bitter mouth,irritability,soreness and weakness of waist and knees,chills and cold limbs,and loose stool(P<0.05).③There was no statistically significant differences in the efficacy of the three groups based on major ocul ar symptom score(P>0.05).Compared with the efficacy based on the total score of TCM symptom,the total effective rate of the joint group,the Chinese medicine group,the western medicine group were 96.67%,90%,and 43.44%,respectively,with statistically significant differences(P<0.01).Conclusion:Wumei pill plus or minus prescription combined with artificial tears has significant effect on the treatment of the upper heat and lower cold syndrome of aqueous tear deficiency dry eye,and can obviously improve ocular and systemic symptoms,and achieve a more precise synergistic curative effect.
