| Objective:To explore the relationship between NT-pro BNP,CysC and TCM dialectical typing of CHF and cardiac function grading by detecting the expression levels of NT-pro BNP and CysC in patients with chronic heart fai lure(CHF).Method:458 CHF patients hospitalized in the Department of Cardiology,Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2015 to June 2018 were selected as the experimental group,The basic data,symptoms and signs,morbidity,tongue fur pulse,medical history,auxiliary examinations and other clinical data were collected when patients were admitted to hospital.Then,according to the characteristics of TCM symptoms,the patients were classified into syndrome types,and the distribution characteristics of different syndrome types NT-pro BNP,CysC and cardiac function were further analyzed.Result:1.There was no statistical difference in baseline data of enrolled CHF patients(sex,age,complications).2.Distribution characteristics of TCM syndromes in patients with chronic heart fai lure:heart-lung Qi deficiency was the first in 132 cases(28.8%),The second is 106 cases of hydroretention with asthenic yang(23.1%),92 cases of blood stasis due to qi deficiency(20.1%),58 cases of syndrome of phlegm and blood stasis obstructing lung(12.7%),deficiency of both qi and yin in 39 cases(8.5%),31 cases of Heart kidney Yang deficiency(6.8%),depletion of yin causing yang collapse in 2 cases(0.4%).3.Distribution characteristics of NT-proBNP of various TCM syndromes:TCM syndromes were correlated with NT-proBNP,With the increase of NT-proBNP,The order of TCM syndromes is:deficiency of both qi and yin,syndrome of phlegm and blood,blood stasis due to qi deficiency,heart-lung Qi deficiency,Heart kidney Yang deficiency,hydroretention with asthenic yang,depletion of yin causing yang collapse.4.Distribution characteristics of CysC of various TCM syndromes:TCM syndromes were correlated with CysC,With the increase of CysC,The order of TCM syndromes is:deficiency of both qi and yin,syndrome of phlegm and blood,heart-lung Qi deficiency,blood stasis due to qi deficiency,Heart kidney Yang deficiency,hydroretention with asthenic yang,depletion of yin causing yang collapse.5.Distribution characteristics of various TCM syndromes and cardiac function grading:deficiency of both qi and yin account for a large proportion of Ⅱ grade of cardiac function;syndrome of phlegm and blood,heart-lung Qi deficiency and blood stasis due to qi deficiency account for a large proportion of Ⅲgrade of cardiac function;,hydroretention with asthenic yang,depletion of yin causing yang collapse account for a large proportion of Ⅳ grade of cardiac function.6.The correlation analysis showed that NT-pro BNP level was positively correlated with CysC level(P<0.01).Conclusion:1.There was no statistical difference in baseline data of enrolled CHF patients.2.Distribution characteristics of TCM syndromes in patients with chronic heart failure:heart-lung Qi deficiency>hydroretention with asthenic yang>blood stasis due to qi deficiency>syndrome of phlegm and blood stasis obstructing lung>deficiency of both qi and yin>Heart kidney Yang deficiency>depletion of yin causing yang collapse.3.Distribution characteristics of NT-proBNP of various TCM syndromes:TCM syndromes were correlated with NT-proBNP,With the increase of NT-proBNP,The order of TCM syndromes is:deficiency of both qi and yin<syndrome of phlegm and blood<blood stasis due to qi deficiency<heart-lung Qi deficiency<Heart kidney Yang deficiency<hydroretention with asthenic yang<depletion of yin causing yang collapse.4.Distribution characteristics of CysC of various TCM syndromes:TCM syndromes were correlated with CysC,With the increase of CysC,The order of TCM syndromes is:deficiency of both qi and yin<syndrome of phlegm and blood<heart-lung Qi deficiency<blood stasis due to qi deficiency<Heart kidney Yang deficiency<hydroretention with asthenic yang<depletion of yin causing yang collapse.5.Distribution characteristics of various TCM syndromes and cardiac function grading:deficiency of both qi and yin account for a large proportion of Ⅱ grade of cardiac function;syndrome of phlegm and blood,heart-lung Qi deficiency and blood stasis due to qi deficiency ac countforalarge proportionof Ⅲ grade of cardiac function;,hydroretention with asthenic yang,depletion of yin causing yang collapse account for a large proportion of Ⅳ grade of cardiac function6.The correlation analysis showed that NT-pro BNP level was positively correlated with CysC level. |
