| Objective:The subject observe the curative effect on chronic cardiac failure(CHF)patients with Heart-kidney Yang deficiency by compound Zhenwu granules combined with western medicine. Evaluate the capability of compound Zhenwu granules by observe levels of monocyte chemoattractant interleukin-6(IL-6),N-terminal Pro-b-type natriuretic peptide(NT-pro BNP) and high-sensitivity C-reactive protein(hs-CRP) in patients’ blood serum.Survey the change of the patients’ cardiac function(NYHA),cardiac failure integral,the symptoms and the sical signs integral of TCM,integral of Minnesota Living with Cardiac failure Questionnaire(MLHFQ) and left ventricular ejection fraction(LVEF),discuss the mechanism of action.Methods:1.The inclusion criteria was made according to heart-failure diagnostic criteria,NYHA cardiac functional grading standard and criteria of diagnosis and therapeutic effect of TCM diseases. 52 patients with certain diagnostic standard were divided into two groups randomly, 26 patients in each group.2.Treatment group was treated by conventional therapy and compound Zhenwu granules.The contrast group was treated by conventional therapy only. Treatment group was treated by conventional therapy and compound Zhenwu granules. 8weeks for a course.3.Contrast the changes on cardiac function(NYHA), cardiac failure integral, the symptoms and the sical signs integral of TCM,integral of Minnesota Living with Cardiac failure Questionnaire(MLHFQ) and left ventricular ejection fraction(LVEF) before and after treatment, Extract the patients’ serums before and after treatment and then record datum of IL-6, NT-pro BNP and hs-CRP by laboratory test.4.Produce a form patient variables by collecting all the relevant data and information. Analyze the data by SPSS17.0 statistically and then present the data with “(?)±s”. Data said in All continual variables were compared with the use of t tests.Ratio comparisons were made using χ~2 test,p<0.05 was taken to indicate statistical significance.Results:1. Observe treatment in the treatment group between before and after:NYHA,cardiac failure integral,the symptoms and the sical signs integral of TCM,MLHFQ integral, LVEF, IL-6, NT-pro BNP, hs-CRP levels, and cardiac failure integral after treatment all improved.The results are not statistically significant.2. Observe treatment in the contrast group between before and after : NYHA,cardiac failure integral,the symptoms and the sical signs integral of TCM,MLHFQ integral, LVEF, IL-6, NT-pro BNP, hs-CRP levels, and cardiac failure integral after treatment all improved.The results are not statistically significant.3. Comparison between the treatment group and contrast group: The treatment group’s NYHA classes after treatment had ameliorate with that of the contrast group(p<0.05).Cardiac failure integral after treatment in the study group obviously ameliorate(p<0.05).The symptoms and the sical signs integral of TCM after treatment in the study group were superior to that of the contrast group(P <0.01). The MLHFQ integral after treatment in the study group had ameliorate with that of the contrast group(p<0.05).The LVEF figure after treatment in the treatment group were obviously better.(p<0.05). NT-pro-BNP figure after treatment in the treatment group were obviously better.(p<0.05).Serum IL-6, and hs-CRP concentrations decreased after treatment in the treatment group more than that of the contrast group and also had statistically significant(p<0.01). And there have no serious adverse reactions between two groups.Conclusion:Cardiac function and quality-of-life of the cardiac failure Patients with Heart-kidney Yang deficiency ameliorate notable treat by Traditional Chinese medicine compound Zhenwu granules combine with Conventional Western Medicine.The drug mechanism should be: 1. Suppress the immune inflammatory reaction.Reduction of myocardial cell injury. Improve cardiac function.2. Reconstruction of inhibition of collagen degradation and matrix. Improve myocardial remodeling. 3.Effect on RAAS system. Reduce the retention of sodium and water.Improving cardiac function. 4. To improve the function of autonomic nerve. Depress sympathetic nervous activity. Reduce myocardial oxygen consumption. |
