| Objective:1.In this paper, through case follow-up,we explore the common treatment methods for traumatized young permanent teeth, and observe the effect of pulpotomy and pulp revascularization in those immature teeth.2.To explore the current situation and clinical characteristics of children’s teeth trauma in this area,so as to provide reference for the prevention and clinical treatment of dental trauma.Method:1.Select patients who underwent pulpotomy due to trauma in the department from September 2018 to January 2020 to discuss the efficacy of marrow amputation in complex crown compromise;from September 2017 to January 2020,the patients who underwent revascularization due to pulpitis after trauma in our department were followed up to analyze the efficacy of revascularization in the treatment of young permanent anterior teeth.2.From September 2017 to January 2020,2 trained postgraduates were selected to conduct investigation and trauma treatment under the supervision of the tutor。Children aged 13 and under, with a history of permanent tooth trauma, without systemic diseases and with complete medical records were selected for on-site questionnaire survey and telephone follow-up surveys. Before the data collection, the patients and their families were fully explained, and agreed, and the data was analyzed with SPSS statistical software.Result:1.20 cases of pulpotomy were followed up for 1-15 months.here were no abnormalities in clinical examination, and the pulp vitality test was positive. At present, the curative effect was judged to be successful.14 patients undergoing revascularization were followed up for 3 to 18 months. The clinical treatment of 12 patients was effective(85.7%),2 patients failed treatment(14.3%),and 2 teeth showed discoloration of the crown, and one tooth showed diffuse calcification of the root canal.Among them, the two teeth with discolored crowns belonged to 2 patients who failed treatment, and 1person’s tooth root was progressively shortened; In one of the effective patients, there was no abnormality in the dental clinical examination and the root development rate was the same as that of the control adjacent tooth. But imaging examination found that the crown sealing material entered the middle and lower canal.Due to the short follow-up time of the case,no one who has a positive pulp viability test has been observed at present.2.Based on the investigation and analysis of 193 valid data(420 traumatized teeth),it is found that most of the trauma occurred in the age of 7-11 years, mostly in males. It was common in the maxillary central incisors, mainly in schools, which was caused by fighting and sports activities between classes.Traumatic teeth involve more enamel and dentin at the same time, and different types of injury have different tendencies in age and cause of injury. In addition, the deeper the parents’ knowledge of trauma and the higher their parents’ education level, the earlier their children’s first oral consultation.Conclusion:1.Pulpotomy is an effective method to maintain pulp vitality after complex crown(root)fracture of young permanent teeth. The short-term efficacy is satisfactory, and long-term efficacy needs to be followed up.Revascularization can be used in the treatment of young permanent anterior teeth with pulpitis due to trauma. The treatment method has a high short-term effectiveness, but the long-term effectiveness still needs to be followed up.2.Considering that children’s awareness of tooth self-protection and their parents’ awareness of oral health are not strong,it is necessary to carry out a preventive education program for parents and school teachers and children in this area to improve the awareness of tooth trauma. |
