| ObjectiveThis study retrospectively analyzed the clinical effect and related influencing factors of endodontic revascularization in the treatment of chronic periapical periodontitis of young permanent teeth to provide reference for clinical diagnosis and treatment.MethodsThe case data who were diagnosed with chronic periapical periodontitis and had completed the pulp revascularization treatment was collected from the Department of Pediatric Stomatology of the First Affiliated Hospital of Zhengzhou University from January 2016 to February 2019.It was recorded in detail of the patient’s basic information,treatment process,clinical examination results and imaging examination results for patients who met the inclusion criteria.And the data was imported into an Excel table for statistical description and analysis.ResultsA total of 43 patients and 43 teeth were included in this study.The average follow-up time was 17.72(12 to 32)months.The clinical symptoms of 42 teeth were disappeared.Among them,28 were successfully treated,14 were effective,and 1 was failed who was performed with apexification.The success rate reached 65.1%,the effective rate was 97.7%,and the failure rate was 2.3% as of the last review.According to different causes,it can be divided into dental trauma group and deformed central cusp broken group.A total of 13 teeth were included in the dental trauma group.The average follow-up time was 15.62(12-24)months and all 12 clinical symptoms were disappeared.Among them,5 were successful and 7 were effective.The success rate was 38.5%,the effective rate was 92.3%,and the failure rate was 7.7%.The root healing types were mainly type I(5 cases)and type III(5 cases).3 teeth showed calcification in the root canal among them.30 teeth were included in the deformed central cusp broken group.The average follow-up time was 18.63(12 to 32)months and all 30 clinical symptoms were disappeared.Among them,23 were successful and 7 were effective.So the success rate was 76.7% and the effective rate was 100%.The root healing types were mainly type I(22 cases).11 teeth showed calcification in the root canal among them.From the perspective of the continued development of postoperative tooth roots,the etiology is the potential influencing factor for postoperative tooth root development.The deformed central apex broken group is more conducive to tooth root development than the tooth trauma group.The difference between the two groups is statistically significant.However,the stage of tooth development,the presence of fistula,the loosening of the tooth and the time of follow-up had no effect on the continued development of the tooth root,and the difference was not statistically significant.As far as the influencing factors of postoperative tooth root calcification were concerned,the etiology,the stage of tooth development,the presence or absence of fistula,the looseness of the teeth and the time of follow-up have no effect on it,and the difference was not statistically significant.Conclusions1.The pulp revascularization in young patients with chronic periapical periodontitis was effective and successful in a short period of time.2.Different etiologies are the potential influencing factors for the continued development of postoperative tooth roots.The postoperative deformity of the central apex fracture group was significantly better than the tooth trauma group,and the clinical effect was better.3.Root canal calcification was the main complication of the experiment,and its related factors are not yet clear. |
